Overall purpose of the role

To support the Assistant Director in delivering the DOH policies related to Continuing Healthcare and Funded Nursing Care, so that the policies are implemented to a high standard which enable consistent decisions to be made on an individual’s eligibility for NHS funded healthcare and then subsequently safe and appropriate clinical care is commissioned monitored and reviewed.
• Responsible for providing professional expertise in relation to CHC, including the legal associated frameworks e.g. S117
• Clinical and operational management responsibility for the Continuing Healthcare (Adults with a Physical Disability and Learning Disabilities) and Funded Nursing Care services to ensure that a high quality and responsive service is provided and monitored within clinical and non-clinical legal governance frameworks (NHS and Private providers).
• To support the development of the commissioning strategy for continuing healthcare and funded nursing care that it is integrated with other ICB commissioning strategies so that contracts are procured monitored and evaluated for high quality and cost-effective services that meet the needs of individuals eligible for NHS funded care.
• Lead on non- CHC pathways e.g. Discharge to assess,
• Ensure all clients are appropriately assessed e.g. Deprivation of Liberty, best interest assessments etc
• To implement the strategic and operational vision for the service area in line with National and Local commissioning strategies and deliver effective demand management systems that contribute to the ICB’s objectives.
• Develop partnership working and support the joint planning and commissioning processes that are developing within the local health and social care economy.
• The post holder will define current processes, facilitate discussions, and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.
• To provide expert advice to the Provider Services and senior colleagues in health and social care on service operational delivery, particularly where Nursing and Allied Health Professionals have a lead role.

Main Responsbilities

- To ensure the service is delivered with the service user at the heart of the assessment process and that the pathway is completed in line with best practice incorporating timelines and eligibility processes and outcomes
• To make eligibility decisions on behalf of the ICB in respect of NHS continuing healthcare processes and multi-disciplinary recommendations ensuring all evidence is present to support the final recommendation and verification
• Support collaborative working across NCL with key stakeholders including, but not exclusively, all appropriate local authorities, NHS Trusts, third party providers, service user and carers groups.
• Assist with public relations and marketing activities.
• Coordinate Freedom of Information requests, keep under review the framework and in accordance with the legislation.
• Overall responsibility CHC teams ensuring high quality service delivery to individuals requiring an NHS continuing healthcare assessment ensuring full compliance with the National Framework for NHS Continuing Healthcare and NHS Funded nursing-care (2018).
• Ensuring the patient pathway is robust and delivered to best practice and national and locally defined standards
• To support the identification and sharing of best practice, clinically and managerially for the delivery of a high-quality NHS Continuing Healthcare service.
• To provide professional support to the Assistant Director and service as a whole
• Ensure prescribed packages of care meet the patients need and are Value for Money.
• Responsible for matching the available budget for each workstream to the appropriate staff resources and projects and for ensuring that the budget breaks even. Co-ordinate the budget, monitoring expenditure against target areas.
• Represent the ICB on Independent Review Panels held and managed by NHS England.
• To be responsible for the management of complaints and appeals including outstanding appeals against previously unassessed periods of care, in line with national policy changes.
• To take the lead in the management of investigations relating to appeals, complaints, and Freedom of Information requests. To ensure an up-to-date log is kept of the same and what stage each is at any moment in time.
• Responsible for supporting and monitoring the commissioning of packages of care and procurement of services to ensure high quality care delivery.
• Support the preparation strategic reports / dashboards and briefings for directors and stakeholders.
• In depth analysis, interpretation and production of complex and multiple reports including financial returns.
• Highlight exceptions and risks ensuring mitigating action can be taken to keep the programme on track.
• Drafting reports summarising status on issues, appraising outcomes, and providing progress reports for the Head of Department
• Delegated responsibility for the provision of a NHS Continuing Healthcare service which is compliant with all statutory duties, Standing Rules and Regulations.
• Ensure the service is delivered on time, to national and local quality standards and in a cost-effective manner, adjusting practices and processes as required.
• To be responsibility for developing policy and procedures that are necessary to ensure the safe, robust, and high-quality delivery of the NHS Continuing Healthcare service, including briefings, commissioning, business, or resources.
• Requirement to be part of the NCL on-call rota.
