In addition to being an expert in your field in possession of extended competencies including vacuum assisted closure & syringe driver management you will need to demonstrate excellent communication skills to illicit an accurate, holistic assessment of patient needs as part of a case management approach. We offer training and support a Health Coaching approach to move towards greater engagement with our patient population regarding self-management particularly in those with long term conditions.
As part of the role you will be required to work within a dedicated referral centre and will possess well developed critical thinking & negotiating skills to aid the decision making & triage process. You will be responsible for managing a defined caseload of patients with delegated responsibilities to your Band 5 nursing team to support continuity of care for patients as part of the nursing model of care. Prior experience with electronic patient record IT systems (e.g Systm One) is advantageous, however full support and training will be given.
You will be keenly aware of the importance of collaborative working and have some understanding of the services and other professions within the newly merged Gloucestershire Health and Care NHS Foundation Trust including our own Therapy colleagues within the ICT (Integrated Community Team), GP surgeries, Rapid Response and Complex Care at Home ensuring the right care is delivered at the right time, by the right professional for patients.

Detailed job description and main responsibilities
As part of place based working you will be keen to get to know your own patch and the Voluntary and 3rd Sector Services which closely align including the Community Wellbeing Service which supports the NHS to provide holistic, person centred care.
You will require current NMC registration as a Registered Nurse, a sound knowledge of evidence based care and best practice, evidence of post-registration development and Your transition into this new role will be supported with a comprehensive induction plan, regular clinical supervision and leadership support from your line manager. You will also join an established, highly experienced peer group of Senior Community Nurses/District Nurses.
A valid UK driving licence plus the use of own transport for business purposes is essential. For qualifying staff the Trust has a Lease Car Scheme in place and also offers a popular Salary Sacrifice Car Scheme with our partner Tusker.
