Job overview
We are looking for a highly skilled and compassionate Registered Nurse to join our Matron Team.
The successful candidate will work with Central Neighbourhood Team to support the residents in this area. This is a permanent full time position, part time will be considered.
It is the expectation that the successful applicant has achieved non medical prescriber V300 certificate.
Advert
To support residents in their own home who are acutely unwell , providing same day response to enable patients to stay at home and prevent hospital admissions where safe to do so. There is potential for case management where you will have responsibility for planning, managing and coordinating the care of those with highly complex needs and long term conditions .
A primary objective will be to work collaboratively with patients in their own homes, the acute hospital and community teams to ensure that residents receive the right care, at the right time in the right place.
We are looking for someone with excellent leadership, organisational and communication skills, and a strong quality improvement focus- as well as the ability to analyse and interpret data.

- A highly skilled senior member of the integrated Neighbourhood team supporting patients within their own homes.
- The post holder will have a clinical role with responsibility for same day assessments using extended skills and independent prescribing to support patients at home. Planning, managing and co-ordinating the care of patients under your care.

Essential criteria
- First level Registered General Nurse – currently registered with the NMC or HCPC registration.
- Educated to Degree or diploma level or equivalent experience.
- Up-to-date evidence of continuing engagement in professional development and of reflective practice in a professional portfolio.
- Non-medical Prescribing qualification – V300
- Consultation and Physical examination
- Prescribing Ability to work within PGDs
Desirable criteria
- Evidence of continuing professional development in chronic disease management, specifically Diabetes and COPD
