Job overview
We are looking for an experience band 6 or band 7 social worker to join out Integrated Discharge Team in Riverside in Hillingdon or Park Royal in Brent.
As a band 6 this will be a developmental role within 6 months to get you ready to be a substantive band 7.
For a band 7 it will be a developmental role in developing skills, knowledge and abilities through multi-agency working with mental health service users in inpatient settings, to remove barriers to discharge.

Detailed job description and main responsibilities

We are looking for a Social Worker to join our Integrated Discharge Team, in Riverside Centre for mental health in Hillingdon or Park Royal Centre for Mental Health in Brent.
We are looking for a motivated, knowledgeable and skilled social worker who can work along side a Multidisciplinary Team in an in-patient, identifying and removing social barriers to discharge.
You will be managing a caseload across a few in-patient wards, whilst having support from a band 4 support worker in managing the caseload.
We are seeking a candidate who has a keen interest in or knowledge of barriers to discharge to service users in-patient settings and who can work to resolve these thorough the discharge planning process.
You will be working with other internal and external agencies E.G: housing, adult social care and immigration.
You must have an in-depth knowledge of social care legislation and associated local authority guidance.
You will be part of the local inpatient team and the trust wide Integrated Discharge Team.
The successful applicant will have direct and indirect contact with service users and a caseload to manage.
You will supervisor responsibility for the worker undertaken by unregistered support workers within the team.
You will be supported by a Service Manager Lead at Band 8A and the Service Lead Band 8B for the Integrated Discharge Team.

Main duties of the job
You will work in a targeted way to achieve the discharge plan for inpatients, maximising independence and choice for the individual, and work in a solution focussed way to ensure that any social barriers to discharge are addressed in a timely manner to support an effective discharge.
You will screen all new admissions to hospital to ascertain if there are any barriers to discharge and will join key meetings to support with discharge planning.
You will work with individuals, families and carers, community services, housing departments, third sector organisations, and other relevant parties to ensure smooth transition to the community from hospital.
This is a hospital based post and requires the successful, applicant to work on site and face to face in multidisciplinary meetings and whilst supporting service users.
You will focus on all KPI’s linked to length of stay and out of area placement elimination.
You will sit organisationally within the Jameson Urgent Care Service as part of the Integrated Discharge Team, however, operationally you will be part of the Inpatient Service team and be present on the inpatient wards within a defined geographical area working with an identified caseload to enable effective discharge planning.
