About
We provide a full range of acute clinical services from Northampton General Hospital and Danetre Hospital in Daventry. We are a designated cancer centre and have recently invested in a number of additional specialist services, including in-patient renal services and interventional cardiology. All elective inpatients and emergency admissions are screened for MRSA. As part of our work to improve clinical outcomes we are taking part in a national patient safety programme and have invested in systems to capture patients views on the service we provide. We understand that sometimes a small change can make a big difference for patients.

MACMILLLAN GENERIC NURSE

There is an exciting opportunity for a passionate, committed, experienced cancer nurses to support site specific nurse specialists, providing cancer nursing support across different cancer sites maintaining continuity of care in the absence of a nurse specialist or when services are under pressure.
The role involves working across different tumour sites undertaking a comprehensive Holistic Needs Assessment and care planning at key stages during the pathway acting as the patient’s main point of contact, coordinating their pathway from the point of diagnosis and during the illness experience.
The post holder will have the opportunity to provide specialist support during outpatient consultations, review inpatients and respond to alerts when cancer patients are admitted as an emergency, demonstrating the ability to assess and manage the individual holistic needs of patients/carers.

Working as part of the Macmillan Nursing Team, the post holder will support the delivery of high-quality patient centred care to those affected by cancer, providing cover to site specific teams that have a reduced capacity of staff that need specialist support to maintain continuity of care for patients.
The post holder will carry a caseload of patients, providing specialist information, help and support, liaising with other healthcare professionals within the cancer multidisciplinary teams, primary and secondary care to ensure a seamless service for patients and their families.
The post holder will be expected to work across different tumour sites, dependant on the needs of the Macmillan service, supporting Macmillan Clinical Nurse Specialists in maintaining a service for cancer patients and back filling for standalone post holders in their absence.
The post holder will be responsible for implementing the key elements of personalised care, ensuring patients are offered a Holistic Needs Assessment/care planning at key stages in the pathway inline with national directives

Work across different tumour sites supporting the Macmillan Nursing Team to maintain continuity of care and a seamless service for cancer patients.
Provide cover arrangements for Macmillan standalone post holders for periods of absence.
To undertake Holistic Needs Assessment at key stages in the patients’ pathway in line with national guidance, using a recognised tool, ensuring their physical, emotional, social, financial and spiritual needs are identified.
Use supportive conversations to develop and implement plans of care to meet the individual needs of patients/carers, ensuring they are reviewed and adapted in response to the changing clinical situation.
To attend the weekly multidisciplinary team meeting as required, contributing to the multidisciplinary discussion and patient assessment/care planning decisions and co-ordinate actions from the MDT.
To provide specialist advice on the management of symptoms, referring those with complex needs to the Specialist Palliative Care Team.
To demonstrate enhanced practice in planning, implementing and evaluating nursing care, documenting the outcomes of consultations in the hospital notes and on the Somerset Cancer Database.
To provide Level 2 psychological support for patients/carers, referring patient with complex needs to the Macmillan Clinical Psychologist.
Act as the main point of contact or responsible for nominating the main point of contact for the patient depending on their individual needs.
To ensure patients receive specialist verbal and written information at the appropriate point in their cancer pathway to meet their individual needs.
To become acquainted with the local and regional care pathways for patients living with cancer, including knowledge of contact points for referral and advice.
To identify and meet the educational needs of those affected by cancer.
Ensure the palliative care needs of patients and their families are identified and met, referring patients with complex needs to the Macmillan Specialist Palliative Care Team.
To support discharge planning working with the Trust discharge team and the ward to ensure a safe effective discharge when the patient deemed clinically fit.
To identify the patient’s Preferred Place of Care at the appropriate point in the cancer pathway, ensuring patients are able to be discharged to their place of choice where possible.
To identify patients suitable for the AMBER Care Bundle.
To ensure patients/carers are supported during significant consultations, acting as the patients advocate.
To ensure effective communication within the team, documenting interactions on the Somerset Cancer Database.
Promotes a culture that supports and enables self-management strategies sign posting patients to the dedicated cancer You-Tube channel.
To work in line with the NMC Code of Professional Conduct
To promote collaborative working relationships and effective communication between all members of the cancer multidisciplinary team and across different boundaries of care.
To provide specialist nursing advice and support to other health professionals related to cancer.
To identity own personal development needs to work across different tumour sites and take appropriate action to ensure that these are met, actively engaging with the learning and development opportunities offered by Macmillan Cancer Support.
To maintain a personal professional profile clearly demonstrating evidence for revalidation.
To participate in the formal review process and plan future personal development and competencies to be achieved.
Make sure that people affected by cancer are aware that they are interacting with a Macmillan professional and know the full range of resources and services available through Macmillan Cancer Support.
To record nursing interventions on the Somerset Cancer Database
To record data to demonstrate the effectiveness of the Macmillan service.
Participate in Macmillan Service Reviews in order to drive service improvements.
Support the implementation of national guidelines/policy related to cancer.
Support the implementation of personalised care, working with the Macmillan Clinical Nurse Specialists and the Macmillan Personalised Care Transformation Lead.
To participate in clinical audit and research initiatives.
To promote best practice ensuring care in evidence based.
