An opportunity has arisen to appoint a new team manager for the Out of Hours Community Nursing Team. The post is full time 37.5 hours per week at pay Band 7.
Based within the Countess of Chester Health Park, the Out of Hours Community Nursing Service is operational between the hours of 5pm and 8.30am 7 days a week and covers a wide geographical area across West Cheshire.
The successful applicant will be willing to work flexibly in order to support the evening and night staff and to link in with the wider care group during office hours.
This is a key role in ensuring people within our care are provided with 24 hour, seamless community care. The team cares for people at end of life and responds to urgent healthcare needs in the out of hours period.
The successful applicant will have great leadership and communication skills, be passionate about delivering care that is person-centred and will have experience of managing a multi-disciplinary team or service at Band 6.

To manage, lead and be accountable for the Out of Hours Community Nursing Service. To enable people to access appropriate and maximise their own, or their families’ independence. Through partnership within the team and effective liaison with the Integrated Team Leaders to develop the Out of Hours Community Nursing Service to:
Effectively assess and support the urgent healthcare needs of adults and older people in the out of hours period.
Promote health, wellbeing, independence and choice.
Continue to develop demand management programmes, preventing avoidable admissions to acute care and facilitating early or timely discharge.
To maximise opportunities for local commissioning and promote inter-agency and multi-disciplinary working across other professional barriers.
To continually review the focus of activities to ensure alignment to core business, strategic aims and improved service delivery outcomes.
