Job overview
This is a Senior Public Health Practitioner role within the Hywel Dda Public Health Team. The team supports the University Health Board and its partners across the counties of Carmarthenshire, Ceredigion and Pembrokeshire. This is a generic role, but in the first instance the post holder will contribute to the Social and Green Solutions for Health work stream through the planning, implementation and evaluation of the projects and programmes across this exciting, innovative and developing public health agenda. These may include:
• Work with partners to identify social and green health objectives within a wider social model of health. These will be captured within an Action Plan and will be mapped against the Wellbeing Goals for Wales.
• Co-ordinate the commissioning of effective funding mechanisms to support grass roots, place based activities and opportunities.
• Co-ordinate the continued development of Social Prescribing across Hywel Dda.
• Link closely with teams within Hywel Dda University Health Board and partners working on green health, sustainability, decarbonisation and one health agendas, ensuring effective cross working.

BAND 7 SENIOR PUBLIC HEALTH PRACTITIONER (Social and Green Solutions for Health)
We are looking for and enthusiastic and motivated person with an interest in developing their public health career to join our team. The Hywel Dda Public Health Team works with Hywel Dda University Health Board and the three local authorities of Carmarthenshire, Ceredigion and Pembrokeshire, and other key partners.
This is a generic post within the team, but initially the post holder will have delegated responsibility for programmes of work within the Social and Green Solutions for Health agenda. This encompasses a range of priorities aiming to support and develop the use of community and nature based opportunities and assets to improve health and well-being, as well as links to climate change and sustainability goals. The role includes project management, strategic partnership working and the management and the delivery of agreed priorities that aim to promote social and green solutions for health and well-being across Hywel Dda.
Travel will be necessary within the Hywel Dda Health Board area and outside the area on occasion. The office base could be either in Haverfordwest, Aberaeron, and Carmarthen.
The ability to speak Welsh is desirable for this post: English and/or Welsh speakers are equally welcome to apply.
