Job overview
Royal Free NHS Foundation trust is recruiting Band 4 Nursing Associate in our community-based dialysis unit based in Tottenham Hale.
We are looking for an Nursing Associate with substantial experience in Haemodialysis and successful completion of either Haemodialysis competencies or equivalent to work as a member of a Nurse led Dialysis unit. As an Nursing Associate you will work as part of the team accountable for the delivery of patient care under the supervision of a registered nurse.
You will be proficient and have experience of initiating & terminating dialysis including cannulation of AV Fistulas. You will have experience in training haemodialysis patients to perform self-care. Your responsibilities will include preparing equipment for treatment, ensure it is kept clean & supplies are well-stocked, safely stored, used efficiently; keeping documentation up to date, provide safe quality care, ensuring the best clinical outcomes and patient experience. Act as an effective role model/mentor promoting excellence through practice.
You must be educated to foundation degree in Health & Social Care or equivalent and have recent experience of working with patients in Haemodialysis area. You will be a good Multi-disciplinary team worker with flexibility. Contribute to the provision of a safe and therapeutic environment for all
patients, relatives, friends and hospital visitors.
To ensure high standards of care are given to all patients, monitoring this through
spot checks and clinical quality indicators outcome measures.
To contribute to patient and relative feedback by undertaking daily clinical rounds
with the aim of providing excellent standards of care.
To collaborate in the assessment of patient care needs and the development,
implementation and evaluation of plans of care, ensuring high standards of
evidence based care.
To ensure communication with patients and significant others is established,
ensuring empathy and reassurance regarding, assessment, care and treatment.
To understand and support safeguarding principles for patients and escalate
any concerns to the registered nurse.
To facilitate early discharge planning being carried out to enable appropriate
arrangements to be in place for patients leaving hospital or transferring in order to
achieve a safe and timely discharge from hospital.
To ensure that all patients are cared for in a clean and well-maintained clinical
area, ensuring that all staff are aware that they are directly responsible for
cleanliness standards when in charge and escalating issues when necessary.

Detailed job description and main responsibilities
Maintain a professional behaviour at all times and promote a positive image of Royal Free London NHS Foundation Trust at all times, in line with World Class Care values.
To be responsible for designated aspects of nursing care for a defined
group of patients under the direction of a registered nurse.
Perform more complex elements of patient assessment from admission
through to discharge. Plan and deliver high standards of care to individuals and groups of patients. (Under indirect supervision from the registered nurse).
To understand and adhere to all relevant trust policies and procedures. To prioritize care as appropriate.
To demonstrate good communication skills when dealing with patients,
relatives and members of the multidisciplinary team utilising sensitivity, empathy and ability to manage conflict.
To immediately escalate a clinical or non-clinical incident to the registered nurse.
To understand the principles of the Friends and Family Test and patient
feedback and how it is used to improve and maintain an excellent patient
experience.
Contribute to effective and timely discharge planning.
To keep nursing staff fully informed and to seek advice and support when
indicated.
To demonstrate initiative and the ability to recognise/identify changes in a
patient’s condition/treatment and report promptly to the registered nurse and medical staff as appropriate.
To liaise with relevant healthcare professionals/departments e.g. dietician,
physiotherapist, equipment library and to keep registered designated nurse fully informed. To perform, accurately record and interpret patient observations -temperature, pulse, blood pressure, food and fluid charts, blood glucose monitoring and urinalysis.
To perform, understand and report patient risk assessments e.g. Nutritional assessment scores, Falls Assessment, Moving and Handling, Pressure Scoring Tool Assessment and Bedside Rails Assessment.
To perform additional tasks (as directed by nursing staff) following
completion of relevant competencies/training.
Act in accordance with the Code of Conduct for Healthcare Support Workers in England.
