Trosolwg o'r swydd
An exciting opportunity has arisen for Healthcare Assistants/Trainees to join our teams at Foss Park both on Minster (Adult male acute) and Ebor (Adult female acute). They are both 18 bedded acute mental health ward which provide inpatient mental health support to those over the age of 18.
The aim of the ward is to continue assessing patients, reviewing and updating treatment plans, our focus is around positive behaviour support and developing support plans that can then follow patients through discharge into their homes.

Working within a person centered approach, you will be part of the multi-disciplinary team assessing and identifying patient care needs, you will need excellent communication skills and be able to form therapeutic relationships with people living with ill mental health . You must be able to show empathy, compassion, understanding and patience.
The post hours will comprise of long days and nights and bank holidays.
The successful candidate must have completed the Diploma in Health and Social Care Level 3 or equivalent or have a willingness to work towards it. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).
The successful candidates who do not have the required qualifications will be expected to enrol on the Healthcare Assistant Framework which will include the acquisition of the level 3 Diploma Health and Social Care, level 2 in Literacy and Numeracy or equivalent and achievement of competencies. You will have a maximum of 2 years to complete this.

Maintains safety, privacy and dignity of all patients in the delivery of patient centred care recognising and respecting differences including spiritual and cultural beliefs.
Responsible for recognising the signs of patient harm, abuse or neglect, including poor clinical practice, identifying issues regarding the safeguarding of children who may be at risk and reporting all such concerns in accordance with Trust Safeguarding procedures.
Contributes to the creation of a physical and psychological environment conducive to the provision of high quality care.
Assists patients in the provision of personal care and encourages independence.
Undertakes delegated activities within level of own competency to assist with the implementation of intervention plans as part of the multi-disciplinary team.
Patients have a range of mental health conditions, may demonstrate behaviours that challenge and may have difficult family situations of relevance.
Completes patient assessment tools and accurately summarises within patient care records.
Participates in the therapeutic engagement and observation of patients as
delegated.
Identifies changes in patients’ presentation and acts, reports and records
appropriately.
Regularly accompanies and provides support to patients away from the clinical area, as delegated and dependent on current risk assessment. This includes social inclusion/community integration/transfer to and from appropriate areas e.g. acute hospitals, dentist etc.
Accurately records and reports physiological observations (e.g. temperature, blood pressure, pulse, weight).
Assists in supporting patients to maintain adequate nutrition and hydration.
Assists in the assessment and management of pressure care.
Contributes to the risk assessment and prevention of falls and undertakes
appropriate activities, as delegated, following a patient’s fall.
Monitors patients, as delegated, in accordance with the Rapid Tranquilisation Policy.
Assists with wound management as directed by the individual intervention plan.
Demonstrates safe physical interventions in the management of violence and aggression as required.
Demonstrates safe moving and handling of patients using equipment as required.

Trust Welcome Day
Here at Tees, Esk and Wear Valley Foundation NHS Trust we want you to have a positive experience when you start your new role with us. To ensure you have great welcome, for all new starters joining the Trust, we offer two start dates per month on the 1st and 3rd Mondays of that Month.
On your start date, you will be invited to our in person Trust Welcome Day at our Flatts Lane Centre, Middlesborough. Your first day will be engaging and informative, it will allow you to meet and network with a range of Trust staff, learn more about us and what we offer. For certain clinical roles, your Welcome Day could then be followed by up to 9 days face to face training. If this is applicable to you, further information will be provided ahead of your Welcome Day.
