NEW NORTHUMBRIA VIRTUAL WARD TEAM
Come and join our rapidly developing Community response Team delivering the Northumbria Virtual Ward service.
This is a fantastic and exciting opportunity to work in one of the most beautiful counties in England and be at the cutting edge of delivering acute care to patients in their own home
The community virtual ward service will provide care to patients in their own home who would otherwise have stayed in hospital
We have full engagement with our hospital consultants who will retain clinical oversight for their patients on the virtual ward to support the team across each specialist area including respiratory, frailty, heart failure and surgery
Complementing and utilising our existing community therapy and nursing teams, our virtual ward team will provide intensive treatment, care and monitoring to patients.
This post may be subject to assessment centre and or psychometric testing.

- To take part in daily virtual wards rounds when on shift.
- To build relationships with the various surgery teams and provide a comprehensive clinical service for post operative patients as part of the virtual ward team with an emphasis on patient centred care.
- To lead on the care of patient in the Surgery virtual wards and to support and community nursing team with their care.
- To be responsible and accountable for the physical delivery of nursing care in the patient's usual place of residence, to a defined caseload by using team management and organisational skills

We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate. Detailed job description and main responsibilities
- To work as part of a team, monitoring patient's progression through the virtual ward and working with the Consultants, Advanced care practitioner's, Nurse Specialists and wider nursing team to support patients.
- Promotion of integrated care across the Trust and Community, ensuring quality of services and maintenance of local and national standards.
- To develop initiatives to improve the pathway of care for patients in the post operative period.
- Ensure accurate documentation and patient records are maintained.
- To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.
