Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Job overview
An exciting opportunity has arisen within the Heart Lung and Critical Care Clinical Group to recruit a forward thinking and innovative Senior Nurse for the Complex Care Agenda.
The successful individual will lead a multidisciplinary team that has responsibility for effective discharge planning including for those with complex comorbidities or social care needs, as well as being the strategic nursing lead for the delivery of older persons care within the Clinical Group.
This is a pivotal role that will support the development of a POPS (perioperative medicine for older people having surgery) service improving the surgical pathway and patient experience for the older person.

The Post Holder will lead the site based Clinical Group Discharge Teams , support the development an effective discharge process across all sites for all patients including older adults and patients with complex co morbidities.
The Post Holder will ensure that older people with complex needs are identified within the clinical areas and working jointly with the clinical staff, ensure that appropriate clinical assessment and management of diverse needs of older patients is met. The post holder will also lead and deliver the ongoing education programme for care of the older person and complex discharge.
POPS (perioperative medicine for older people having surgery) is a service that supports older people and those with complex medical problems who need an operation, there is an established POPS service that operates across the Guys and St Thomas Hospital Campus, the post holder would be responsible for implementing this service across the Royal Brompton and Harefield Hospital Campus.

The Heart Lung and Critical Care Clinical Group consists of all services across the Royal Brompton and Harefield Hospitals as well as the Cardiovascular and Critical Care services at the Guys and St Thomas Hospital Sites and is the largest specialist heart and lung centre in the UK and among the largest in Europe.
The Complex care team is a newly formed team led by a head of Nursing to support the complex care needs of some of our older and most vulnerable patients
Detailed job description and main responsibilities
The post holder will work closely with staff of all professional backgrounds across the clinical group for the benefit of patients and to ensure safe discharge for complex patients.
The role will lead on improving discharge planning with the ward staff to ensure patients & families are ready for discharge, aware of ongoing care they may be required and are discharged with all the required information and medication in a timely way.
To act as Lead for Discharge & Complex Discharge for Heart Lung & Critical Care (HLCC) clinical group – provide strategic planning, development and implementation of National guidance, e.g. National Service Framework for Older People, and lead on delivering related Trust strategies across the clinical group.
Lead on or work with:-
- Efficient discharge planning ensuring patients & families know when patients’ have a proposed discharge date, plan and discharge requirements for ongoing care
- Planning specific service improvement such as morning discharge alongside Matrons and ward staff.
- Development projects, initiatives and liaising with a wide range of staff internally and externally, taking responsibility for outcomes, reporting and assurance.
To co-produce Trust policy and deliver locally across all clinical areas within the HLCC clinical group.
To work with the Older Persons’ Team for patients having surgery (POPs). To improve the identification of older people with complex needs, supporting ward staff with the clinical assessment and management of diverse needs of older patients and encouraging them to proactively liaise and refer to other members of the Multidisciplinary Team (MDT) as appropriate.
To lead on the development, monitoring and review of care plans for older patients with complex needs e.g. dementia, history of falls. To lead on planning, development, delivery, and evaluation of training for ward staff and other members of the team to support provision of quality client-centred services for our older patients and those with complex discharge, within HLCC and Trust wide. To be responsible for the reporting of relevant data internally within HLCC and the Trust as well as
To take responsibility for the co-ordination of complex discharges, to support patient flow, across all sites within HLCC; this will be achieved by leading and managing the services for complex discharge including NHS continuing health care co-ordination, working with colleagues in complex discharge, occupational therapy, welfare rights, ward discharge nurses and the lead for adult safeguarding as well as linking with external partners. To lead regular audit of current practice and development and implementation of recommendations. It will include collation of service data for the clinical group and development of dashboard data. To lead the HLCC discharge improvement group meeting, a group of multi professional staff from across the clinical group, to deliver quality improvement activities to improve discharge of patients.
