Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. We provide a full range of hospital and community services for people in south London and as well as specialist care for patients from further afield including cancer, renal, orthopaedic, respiratory and cardiovascular services.
Guy’s is home to the largest dental school in Europe and a £160 million Cancer Centre opened in 2016. St Thomas’ has one of the largest critical care units in the UK and one of the busiest emergency departments in London. It is also home to Evelina London Children’s Hospital.
Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.
In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. You will be responsible for the day to day management of a site wide bed management service seven days per week and will be expected to undertake flexible day time shift work to achieve this.

The successful applicant will be a dynamic individual with a good understanding of patient flow, as well as having experience of cardiothoracic surgery/medicine. and multi-disciplinary working.

Detailed job description and main responsibilities
The Patient Pathway Co-Ordinator will be responsible for the admission of elective and none-elective admissions to Harefield Hospital, supporting the patient pathway and improving communication between multidisciplinary teams.
This role will:
- Maximise available bed resource hospital wide encouraging shared bed information between services.
- Responsible for the day-to-day management of a comprehensive high quality and effective bed management service seven days per week.
- This role includes regular weekend / out of hours working including early / late shifts depending on service needs.
- Establish and work towards a hospital wide view of bed resource with a clinically informed view of the use of available resource, depending on the needs of the service.
- Monitor and oversee Inter Hospital Transfer (IHT) admissions from the NW London Cardiac Network, ensuring pre-procedural workup is completed before transfer is arranged.
- Devise and maintain necessary audit / performance data with emphasis on the efficient patient pathway (e.g. CUR)
- Work as an integral part of the scheduling team to ensure shared knowledge of patients on site ready for surgery, IHT and MDT outcomes.
- To work in close liaison with consultant, registrars, SHOs, nurse practitioners, discharge nurses and ward sisters for discharge planning, to ensure the pathway is improved and implemented.
- Maintain and develop communication and relationships with receiving centres in order to coordinate timely repatriations.
- The demonstration of effective communication and building trust within hospital staff.
- To communicate effectively and work effectively as part of the multi professional and inter-disciplinary team. As well as working independently, using skills to run and effective service
- To monitor admissions out of hours to ensure the protocol is applied.
- To understand when and how to escalate any potential issue either clinical or non-clinical in the best interests of patients, relatives and staff
- To have a full working knowledge of the DATIX incident reporting system and use as appropriate
