We provide a full range of acute clinical services from Northampton General Hospital and Danetre Hospital in Daventry. We are a designated cancer centre and have recently invested in a number of additional specialist services, including in-patient renal services and interventional cardiology. All elective inpatients and emergency admissions are screened for MRSA. As part of our work to improve clinical outcomes we are taking part in a national patient safety programme and have invested in systems to capture patients views on the service we provide. We understand that sometimes a small change can make a big difference for patients.

Team Lead role for site an exciting opportunity to build on your skills gained within the clinical site management team.
This role is part clinical and part management with dedicated hours for management. Job share is available.
You will undertake all HR, sickness, vacancy, datix and budget requirements for site team and night practitioner team.
Ensure team are supported and their mandatory skills are maintained to appropriate level.
Liaise with internal and external stakeholders across all levels effectively.
You will be a valued team member who works with a diverse range of staff both internally and externally.

The Site Team Manager will be responsible for the efficient and effective operational management of the clinical site management team and the night practitioner team. Lead on the implementation of high quality discharge planning processes throughout the Trust.
Their remit is to lead, support and develop the teams’ within their remit to ensure that appropriate, clinically based decision making 24/7 across the trust in regards to none elective admission’s and elective admissions maximising our bed base and fully aligning to right patient right bed ethos. Ensure best practice in planning safe and timely patient movement and transfers in order to achieve best outcomes for patients, support the clinical staff with highly complex patient needs overnight and optimise hospital bed utilisation.
The post holder will work alongside their team and key stakeholders in ensuring that 24/7 the Trust is managed in a consistent and safe manner adhering to trusts policies and guidelines with emphasis on ensuring safety of our patients is paramount
To lead the development, implementation, management and continual improvement of systems, structures, personnel and processes relating to safe patient flow throughout the Trust, ensuring outcomes that optimise patient safety and experience; minimising clinical risk.
To provide interface between the Trust and external stakeholders regarding internal and external pressures, impacts on patient flow.
They will lead on the development of effective links and relationships with relevant professional partners both internal and external to the Trust, to ensure a consistent approach across the trust and external communications.

Discharge Planning and Management
- Active direct led role in the participation in relevant patient flow committees (internal and external), actively sharing information, attending conferences and meetings including a strong commitment to the development of proactive and timely patient flow
- Active oversight and management of non-elective admissions from Emergency Department with view of supporting the four hour standard and adhering to right patient right bed ethos
- Define and communicate local patient flow objectives that will ensure the achievement of the organisational strategy within resource limitations. Communicating highly complex information, negotiating where required to achieve the required outcomes
- With delegated responsibility from the Deputy Chief Operating Officer take ultimate responsibility for effective service delivery, implementing controls to monitor, evaluate and report upon the standards of delivery within bed and site management. Developing and implementing any policies that may be required
- Promote a culture of accountability, openness to change; respect for the individual and team work that supports the achievement of the National Health Service Strategy
- Foster and maintain effective working relationships with Infection prevention and control, ED,ICB and EMAS
- Proactively “challenge” in a constructive way current working practices offering expert advice and guidance as necessary to promote best practice and a sustained improvement in patient flow planning and management
- Be responsible for ensuring that team are aware of any new internal, local, regional and national directives that impact on current bed and data management e.g. new OPEL triggers. Including playing a key role in the interpretation of any new guidance
- Develop and implement a risk management strategy for periods of crowding and ambulance offloading delays in conjunction with the Urgent and Emergency Care Divisional leads, matrons and the external partner agencies.
- Work with the local partner agencies to ensure that the NGH discharge management process meets the service requirements expected by the national, local governing bodies and community providers
- Ensure that the SITREP and any other national or local reports are accurately compiled and dispatched in accordance with the required timeframes.

- Maintain a budget in line with budget setting for the overall Site Team
Organisational Development
- Work within, and provide support to, diverse teams, demonstrating sound leadership capabilities in discharge planning and management processes, to improve the efficiency and effectiveness of the service.
- Be recognised as a leading change agent within the organisation and within the team
- In conjunction with the Site management team maintain and implement a Trust wide communication plan for discharge planning and management across the Trust which ensures that the principles and application of the process are widely understood and owned
- Undertake any necessary presentations to relevant teams within the organisation in order to promote the principles of good discharge planning and management and where appropriate to use the opportunity to promote success and the associated leaning.
- Encourage a climate that allows all staff to question and redefine how work is undertaken ensuring an improved patient focus and cost effective use of resources remains central to the process
- Promote a positive change management culture within the Trust and develop the skills and knowledge of staff
Line Management, Education Training and Development
- Line manage the Discharge team, ensuring all the team receive annual appraisals, addressing training requirements in accordance with Trust policy
- Develop and deliver an agreed programme of training relating to discharge planning and management, to equip staff with the necessary skills to ensure high standards of discharge planning and management are universally achieved by all professional groups across the Trust.
- Provide the clinical areas with a range of training materials to ensure that all staff has access to the current discharge planning and management information regardless of their preferred method for learning and development.
- Coach and support new or junior team members, and any of the staff within the Divisional teams if required
- Ensure members of the Discharge Team receive annual appraisals, addressing training requirements in accordance with Trust policy
- Develop and implement own Professional Development plan in conjunction with the Deputy Chief Operating Officer.
Information & Research
- Actively research examples of best practice related to Discharge planning and management from other organisations and provide a range of options as to how these could be adapted for application within NGH as appropriate
- Contribute to research undertaken by the department by carrying out specific tasks in analysis and presenting outcomes at national conferences as required
- Using highly complex information, benchmarking and best practice evidence bases assess priority areas for action
- Accurately analyse data and disseminate findings adapting its presentation to meet the needs of differing audiences.
