GP with Extended Role (GPwER) - Manchester Case Management

We are looking for a salaried General Practitioner for the Central Manchester Case Management (MCM) service.

MCM is an exciting service developed in Manchester, building on the successful High Impact Primary Care (HIPC) pilots and Active Case Management services.

We are looking for person centred experienced GP to join our friendly supportive team. MCM is a dedicated multi-disciplinary service pro-actively supporting people at high risk of hospital attendance and admission with a range of complex physical health, mental health and social needs. The skilled MDT team share the care of the person, and provide intensive and flexible support. The MCM team includes nurses, social workers, pharmacists mental health and substance misuse professionals team manager and admin support. MCM service works with the wider Integrated Neighbourhood Teams (INT) to draw in local community assets and services as part of the offer to MCM clients.

The aim of MCM is to improve the care of medically, socially and psychologically complex patients, taking an holistic approach, to provide excellent patient-centred health led care, to reduce their risk of hospital admission, improve their quality of life and enable them to better manage their health and wellbeing. The MDT will also liaise with specialist community teams such as the IV team, the Heart Failure team, the Respiratory team to provide best care for the patient.

• Provide clinical oversight, leadership and support to the MCM service and other team members to ensure patient care is safe and effective.
• Provide teaching and training to the team, some of whom are trainee ACPs (advanced clinical practitioners) and nurses and pharmacists undergoing training to become prescribers.
• Collaborate with the patient’s own GP Practice and hospital services, as required, to ensure a clinically safe and shared care approach.
• Keep yourself up to date with the relevant fields of medicine, clinical care and General Practice, comply with NHSE regulations concerning appraisal and revalidation.
• Liaise with secondary care consultants and the patient’s GP as necessary.
• Keep up to date with the relevant fields of medicine, complying with NHSE regulations concerning formal GP appraisal and revalidation. Participation in an annual individual performance review at service level will also be undertaken within the service
• Be up to date on the relevant IT systems, with the ability to order relevant tests etc. 
• Ensure excellent clinical record keeping; recording clear and contemporaneous consultation notes both written and electronic to agreed standards.
• Be aware of and comply with all relevant local service policies/guidelines, prescribing, confidentiality, data protection, health and safety. This will be provided during induction.
