Job overview
An exciting opportunity has arisen within the 18 Week RTT Team; we are recruiting to a part time Band 4 Pathway Navigator post to join our team.
The role of the Pathway Navigator requires an enthusiastic, well organised self-starter, who is able to work independently and as part of a team; conversing fluently, logically and confidently with multidisciplinary personnel within the Trust.
As part of clinical board 8, the 18 Week Pathway Navigator will validate data to ensure a true representation of a patient’s treatment pathway is recorded and reported. This requires the formation of close links with Directorate teams in order to expedite patient pathways/treatment. The Navigator is responsible for providing accurate and timely data, in order to identify patients who may breach national targets and take appropriate action to negate this.
In addition, in order to ensure the Trust provides excellent care without unnecessary delay the 18 Week Pathway Navigator must carry out pathway mapping, analyse delays and bottlenecks and review processes and workflows. The Navigator is also responsible for proactively providing bespoke small group and lecture theatre training.

There is currently significant pressure on NHS waiting lists and a backlog of patients waiting for hospital treatment.
The role requires an enthusiastic, well organised self-starter, who is able to work independently and as part of a team, converse fluently, logically and confidently with multidisciplinary personnel within the Trust to provide excellent care without unnecessary delay.
This post is fundamental to helping the Trust monitor its compliance with NHS waiting time targets and work towards reducing these to pre-pandemic levels.
- The post holder will be allocated a Directorate(s) to work with and will be responsible for the pathway validation of this area, including long waiter meetings with the Directorate to identify delays in patient care and to expedite appointments, investigations, and admissions where possible.
- The post holder will be expected to work autonomously, managing their own workload, meet tight deadlines, manage competing priorities and work in collaboration with the 18-week team to help ensure data is validated for submission against national and local waiting time targets.
- The post holder will be expected to help support the delivery of referral to treatment training within the organisation.
- The post holder will be expected to help support the 18 week team to produce weekly / monthly / ad-hoc reports required by the organisation and external agencies.

Detailed job description and main responsibilities
- Patient Navigators will play a pivotal role in supporting the directorate teams by navigating and tracking patients along their elective journey.
- There will be a requirement to co-ordinate hospital appointments, investigations and treatment, whilst identifying patients who may breach national waiting time targets.

communicating with the relevant management teams.

Essential criteria
- NVQ level 4 or equivalent experience
- IT qualification such as ECDL or Microsoft Office qualification (or equivalent experience)
Knowledge
Essential criteria
- Knowledge and understanding of the Department of Health “Referral to treatment” 18 week guidelines and Department of Health Waiting Time targets
- Knowledge and experience of hospital PAS system
- Experience with data collection, validation and reporting
Desirable criteria
- Previous experience in teaching / training / presentation skills
- Demonstrate experience identifying potential problems and resolution
