Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Job overview
You will be the named Speech & Language Therapist within the newly funded, dedicated therapy service to adults with chronic and severe respiratory conditions referred to the specialist Lung Volume Reduction (LVR) MDT at Royal Brompton Hospital.

You will be responsible for leading the assessment, treatment and management of swallowing and voice disorders associated with LVR interventions (bronchoscopic and surgical) for advanced lung disease at all stages of the patient pathway: pre-operative, post-operative and follow-up. In addition, you will coordinate the SLT service to inpatients within the Respiratory Medicine service. In-depth knowledge and skills alongside significant experience of managing speech, voice, swallowing and upper airway disorders in advanced respiratory disease is essential, including patients requiring NIV and TIV for respiratory muscle weakness. Post-operative care for some patients will include a critical care admission, so the ability to work with tracheostomy and mechanical ventilation following cardiothoracic surgery is needed. Completion of competency-based learning and substantial experience with instrumental dysphagia assessments is required to be able to act as a Lead VF Clinician, Level 2b FEES practitioner (or equivalent), and perform nasendoscopy. Familiarity with using MBSImP in practice is necessary. As this is a new service, skills and experience in leading and evaluating healthcare services is required. On occasions, you be asked to work at Harefield hospital as required by service needs.
Working for our organisation
The LVR service sits within the Respiratory Medicine team, made up of a large and dynamic group of physiotherapists, occupational therapists, dietitians, psychologists and therapy assistants. You will work collaboratively with your multi-professional colleagues and the adult speech and language therapy team to deliver a responsive, patient-centred, high-quality and effective service to patients with a range of respiratory conditions. You will receive operational management from the Band 8a Therapy Pathway Lead (Clinical Specialist Physiotherapist) and professional management from the Band 8a Clinical Lead Adult Speech & Language Therapist. Continuous professional development will be provided through regular clinical supervision, line management, 121s, appraisal, inservice training and access to study leave funding for external learning opportunities.

Essential criteria
- Lead VF clinician
- RCSLT 2b FEES assessor or equivalent
- Competence in performing flexible nasendoscopy
- Completion of RCSLT tracheostomy competencies including the critical care subsection
- Experience of working with patients requiring invasive mechanical ventilation, NIV, TIV and HFNO
Desirable criteria
- Knowledge and experience of managing iatrogenic vocal fold movement impairment following cardiothoracic interventions
- Experience of working with patients with acute and long-term conditions affecting respiratory, cardiothoracic and neuromuscular function, including ARDS, ECMO, MND and muscular dystrophy
- Familiarity with using MBSImP in practice

- In-depth knowledge and experience in assessing and treating communication and swallowing disorders in adults
- Independently able to carry out formal (standardised) and informal (non-standardised) assessments, analyse and interpret findings with a high degree of accuracy to inform clinical reasoning

- Familiarity with upper airway disorders including laryngeal hypersensitivity and chronic refractory cough
