Detailed job description and main responsibilities

To complete high quality reviews efficiently and within national timescale requirements and where necessary support multi-agency reviews which may be of a complex nature

• To adequately support bereaved carers and families with whom they come into contact through the course of undertaking a review

• To conduct both initial and focussed reviews, working collaboratively with the review team and other partners. LeDeR reviewers are expected to work collegiately and provide peer support to other reviewers, particularly around their own area of expertise and supporting progression to completion

• Work with others to ensure effective processes are put into place to streamline the completion of investigations/reviews where processes have the potential to overlap (for example Child Death Overview panel, Safeguarding Adults Review, Coroner’s Inquest).

• Ensure that Safeguarding principles are considered and applied within all elements of the LeDeR programme and establish links with key local Safeguarding contacts and ensure that any concerns are reported as per Safeguarding Children and Adults policies.

• Support the development of local LeDeR arrangements, working alongside the Local Area Contact

• Identify the most appropriate evidence available including the conversation with the family or loved one, conversations with the GP or GP records and other health and care data sources and using high level professional judgement and experience to determine and agree with Local Area Contact whether a review proceeds to a focused review.

• Explain the parameters of the LeDeR review, signpost to other processes such as complaints processes, and discuss with the family their expected outcome of any LeDeR review setting realistic expectations at the start.

• Present review findings at governance panels and actively participate and support the panel to define SMART actions offering professional challenge and proposing solutions to address recommendations identified in completed LeDeR reviews, to ensure that the actions that are developed will achieve the needs of the findings of the review.

• Agree with LACs (and senior reviewers where necessary) any actions coming out of initial reviews (these will be limited and infrequent).

• Report on progress/achievement against agreed actions detailed within the

LeDeR Steering Group Action Tracker

• With support from the LeDeR Steering Groups support a communication and briefing approach in sharing lessons learned from mortality reviews both locally and nationally for dissemination across the health and care in Staffordshire and Stoke-on-Trent.

• Share, present and submit the learning from completed reviews with the local teams and other partners to support identification of learning that can be actioned. To collate and analyse information from completed reviews and feedback to local groups and other forums as required.

• Have the ability to convey and present complex, sensitive and possibly contentious information to a range of audiences - this information includes, but is not limited to, feedback to and from families of deceased clients as well as learning and feedback from reviews which may challenge system partners and local groups.

• Build and maintain relationships with key personnel, including social care and other health care providers, to ensure that reviews are fully informed and that knowledge and information is shared across internal and external stakeholders to improve Learning Disability and Autism care across Staffordshire and Stoke-on-Trent.

• Ensure reviews and associated outcomes are delivered on time to quality standards.

• Encourage and promote a culture that values openness, honesty, rigour and challenge.

• Maintain high standards of documentation and adhere to the national LeDeR programme requirements.

• Be responsible for escalating concerns regarding gaps in provision. Link with, escalate and refer concerns to other processes where necessary and advise families where another process (for example a Trust complaints process) would be more appropriate.

• Analyse and evaluate a range of evidence, some of which will entail reviewing complex, confidential and detailed reports, requiring high levels of concentration.

• Actively support and contribute to the development of key performance indicators for the successful assessment of performance of LeDeR in Staffordshire and Stoke-on-Trent.
