Job overview
An exciting opportunity has arisen for Urgent Community Response Team previously known as crisis response team (UCR) Band 7 Nurse/Paramedic based at Hastings. You will provide an urgent 2 hour response to patients in crisis and where an unplanned admission can be avoided.
This job initially will be with UCR, then shift pattern is subject to change when UCR and Integrated Night Service (INS) merge together to become 24 hour team.

You will provide case management and intensive support to patients in crisis, at high risk of unplanned admission for up to 72 hours and also required to work as a team with Virtual ward staff. You will support the UCR admission avoidance sections of the ambulatory care pathways for UTI, pneumonia and cellulitis including the initiation and administration of IVs and you will provide support to the prevention of unplanned hospital admissions, responding to patients who require urgent assessments and in collaboration with the patient and community integrated team, set up management plans to enable the patient to stay at home where possible.

Person specification
Qualifications
Essential criteria
- Degree/diploma in paramedic science
- HCPC registration/NMC registration
- RN first level nurse (Adults)
- 1st level degree
- Teaching and Assessing qualification
- Non medical independent prescriber / supplementary prescribing, or willingness to undertake training within 12mths of coming into post
- Evidence of continuing professional development in a relevant area of specialist practice to Masters equivalent level
Desirable criteria
- Community Nursing Qualification / Advanced Nurse Practitioner Qualification
- Relevant Masters level degree
- Trained to interpret microbiology and pathology results
- Case Management
- Advanced physical assessment skills
- IRMER trained
Experience
Essential criteria
- Experience of working as an autonomous practitioner in either an acute or community environment
- Good understanding of working in partnership with statutory and non statutory agencies to support patient centred care
- Multi-disciplinary and multi agency working
- Proven extensive experience of working with adults/older people
Desirable criteria
- Experience in a Community setting, or gateway area
- Experience of working between other health and social care organisations /settings
- Ability to interpret blood results
Skills
Essential criteria
- Able to demonstrate effective leadership and organisational skills
- Advanced clinical decision making skills to support patient centred care
- Advanced clinical skills ie cannulation, administration of IV medication
- Advanced physical assessment skills or willingness to undertake within 12 months
- Good knowledge of anatomy and physiology
- Evidence of ability to maintain and monitor high standards of care
- Ability to communicate complex clinical patient presentations to medical colleagues, in particular G.P.’s in Primary Care
- Evidence of ability to direct and co-ordinate complex programmes of care working autonomously and collaboratively across organisational boundaries

- Knowledge relating to long term conditions
- Ability to influence and manage a variety of change management programmes in embracing and implementing change to provide high quality health outcomes
