Job overview
The NHS Long Term Plan contains a key commitment that ‘by 2023/24 children and young people with a learning disability, autism or both with the most complex needs will have a designated keyworker. Initially, keyworker support will be provided to children and young people who are inpatients or at risk of being admitted to hospital. Keyworker support will also be extended to the most vulnerable children with a learning disability and/or autism, including those who face multiple vulnerabilities such as looked after and adopted children, and children and young people in transition between services."
The keyworking model, led by NHSE/I, has been developed through extensive consultation with parents, carers, young people and other stakeholders.
The Islington and Camden Keyworker Service provides coordination and direct support to children with learning disabilities and/or autism, who also have mental health needs and/or behaviours that challenge. This project uses intensive, direct interventions with the aim of supporting children and young people with neurodiversity to remain living at home, in the community; preventing avoidable admissions to inpatient mental health services and/or 52-week residential placements; and to reduce the length of inpatient admissions, supporting early discharges.
Advert
This role will provide high quality, specialist support to children and young people and their families/carers; working as part of a team to help unblock the system, provide a bridge across different parts of the system including health, education and social care, acting as a key point of contact for families/carers. You will be part of a large and diverse multi-disciplinary team, with colleagues from both health and social care; and will work closely with colleagues, teams and services across the professional network, working to improve outcomes of our children, young people and their families.
You will be a strong and passionate leader who is able to be an excellent role model and motivate staff with a proactive attitude. As Deputy Manager you will provide monthly supervision, intervention planning and general support to a small group of staff. You will also hold a small and varied clinical caseload, you will work both independently and in partnership with professionals across the network, providing direct, intensive, clinical support for families/carers. The role will work across various settings, including in homes, schools and in the local community.
You will benefit from high level, expert support and line-management and clinical supervision and weekly reflective groups. You will receive multiple, specialist training opportunities including Positive Behaviour Support, Motivational Interviewing and Non-Violent Resistance.

Detailed job description and main responsibilities
MAIN DUTIES AND RESPONSIBLITIES
- Clinical:
a. To be a key point of contact for families/carers allocated to you, identifying gaps in their support and enabling the system to work together to bridge those gaps.
b. Facilitate the provision and implementation of tailored behavioural and emotional support to help a child or young person and their families/carers develop skills for progress and self-management to meet their agreed goals and outcomes.
c. To support the service to deliver intensive time limited interventions which achieve positive change for children, young people and their families, based on shared objectives and that are regularly reviewed by the Team Around the Child.
d, To work closely with inpatient settings, local CAMHS providers (including crisis teams), social care and education to ensure that the system is working in a coordinated and joined up way for the young person and their family/carer and helping to reduce any blockages to accessing that support.
e. To work with the professional network in planning support, care and treatment in order to facilitate early discharges from hospitals.
f. To work with CAMHS colleagues in the assessment and management of mental health risk.
g. To ensure appropriate service and team representation and input to wide variety of service and clinical meetings as required ie: CETRs; CPA meetings; CIN meetings; CP conferences, EHCP reviews etc.
h. To attend, contribute to, and update on cases at the local Dynamic Support Register meetings, and all other relevant panels and boards.
i. To provide advice and consultation where appropriate, to other health, social care, voluntary sector and education staff working with the client groups.
j. Communicate clearly and sensitively with the child, young person and their family/carer to help them understand and navigate the system.
k. To provide reports and communicate in a skilled and sensitive manner concerning the assessment, formulation, treatment plans and needs of clients.
l. To liaise with other health, social care and education staff, from a range of agencies, in the care provided to clients from each service.
m. To carry out clinical work in health and education settings, and clients’ homes to best meet the clients needs and in line with service practice.
2. Teaching and Training:
a. To continue to develop skills in the area of training and clinical supervision.
b. To complete all trainings as needed and required by Keyworker Service and as agreed by Line Manger in eg: NVR, MI, PBS.
c.To develop skills in providing teaching, training and consultations to wide range of professionals and partner agencies.
d.To ensure the development and maintenance of the highest standards of practice, through active participation in internal and external CPD training and development programmes, in consultation with the post holder’s professional and service managers.
3. Management, policy and service development:
a. To contribute to the development, evaluation, implementation and monitoring of the operational policies and procedures of the LD + Autism Keyworker Service.
b. To establish and maintain good working relationships with stakeholders including partners and parents, to promote joint working and their involvement in the service.
c. To contribute to meeting key performance targets assigned to the service, the collation of management information and financial information to inform monitoring, future planning and cost projections.
d. To be involved in the ongoing development of the service, including making suggestions for improvements and working to implement changes to their service as agreed with commissioners.
4. Research and service evaluation
a. To utilise theory, evidence-based literature and research to support evidence-based practice in individual work and family work and with other team members and inter-agency colleagues.
5. General
a. To work as part of the team, and ensure the team operates in a positive, reflective and forward-thinking culture.
b. Maintain the highest standards of practice, competence and conduct at work, adhering to the code of conduct of the relevant professional body eg HCPC + NMC
c. To take responsibility for promoting and safeguarding the welfare of children and young people in your care, and for those who you come into contact with, responding to any concerns promptly in line with local and national guidance on safeguarding children.
d. To work flexibly and with initiative to meet the core aims of service delivery to clients.
e. To contribute to the development and articulation of best practice within the service area, by exercising the skills of a reflexive and reflective practitioner, taking part in regular professional supervision and appraisal and maintaining an active engagement with current developments in the post holder’s area of specialism and in child mental health and related disciplines.
f. To maintain the highest standards of clinical record keeping including electronic data entry and recording, report writing and the responsible exercise of professional self-governance in accordance with Service and Trust policies and procedures.
g. To maintain up to date knowledge of legislation, national and local policies and issues of relevance to the service and client group.
h. To undertake such other duties and responsibilities, appropriate to the grade of the post, as may be agreed with the service managers.

Essential criteria
- 2 years’ experience of working in a service for children and/or young people at a minimum Band 6 level (or equivalent).
- Experience of working as a keyworker/case manager and holding a caseload of patients.
- Experience of providing structured support, delivering positive activities and ensuring the achievement of agreed individual outcomes.
- Experience of working with children or young people with neurodiversity, and excellent skills in engaging, communicating and consulting with neurodiverse cyp and their families/carers.
- Ability to deliver good outcomes for children or young people with neurodevelopmental differences and family/carers, taking a holistic approach working with the Team Around the Child to maximise potential, increase family resilience and reduce reliance on statutory services.
