Detailed job description and main responsibilities

The Clinical Nurse Specialist will:

Put quality at the heart of practice by delivering evidence based individualised and personalised care through holistic needs assessment, planning and evaluation of care and all care interventions.
Work at a highly specialised level, practicing autonomously as well as within a multi-disciplinary team and are self-directed.
Actively integrate theory and practice.
Act as a Key Worker for a clinical caseload, working to coordinate care and provide easy access for ongoing support for patients.
Assess people’s information needs and provides relevant information to meet those needs.
Enable people to access, identify, appraise and interpret information.
Plan, prioritise and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, providing direct support to patients and clients.
Lead the nursing contribution to service development, ensuring the pursuit of excellence in care.
Use professional judgement in managing complex and unpredictable care events.
Draw upon an appropriate range of multi-agency and inter-professional resources in their practice.
Appropriately define boundaries in practice.
Promote a high standard of nursing by initiating and co-ordinating the assessment, planning, delivery and evaluation of the holistic needs of patients / families through evidence-based practice following agreed policies, protocols and guidelines.
Practice a high standard of communication including highly sensitive and complex information to develop and sustain partnership working with individuals, groups, communities and agencies. Utilise evidence-based information through a variety of communication media.
Maintain adequate patient documentation to NMC requirements for all patients seen and advice given in any practice setting and contribute to clinical activity / data collection as required.
Support patients in the pre and post-operative phase to be able to achieve a maximum state of health physically, psychologically and emotionally.
Promotes self-management, undertaking risk stratification to determine those who can self-manage, following education, those who will need guided support to self-manage and those who will require on-going, face to face support.
