Job overview
To work in isolation or as part of a multidisciplinary team to provide high quality, patient-centred, timely and cost-effective care. All specialist nurses provide tailored care depending on the patient’s level of need. They also provide education and support for patients to manage their symptoms, particularly patients with long term conditions and multiple morbidities. RCN (2014)
The post holder will:
- Assess individuals, families and populations holistically using a range of different methods, some of which may not be usually exercised by nurses such as physical assessment and history taking or advanced health needs assessments.
- Ensure the provision of continuity of care.
- Assess and evaluate, with patients and / or carers, the effectiveness of treatment and care provided and make changes as needed.
- Work autonomously within a wider health care team and as a lone practitioner.
- Work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health, outcomes and healthcare delivery systems.
Advert
Work with the Advanced Therapist Practitioner to run a comprehensive pre-operative and post-operative enhanced recovery service for patients undergoing thoracic surgery:
- Make contact with all patients having major thoracic surgery either at their out-patients appointment or as soon as possible afterwards to introduce them to the enhanced recovery programme and ensure they have the appropriate level of support.
- Make sure that people affected by cancer know about the full range of resources and services available through Macmillan.
- Provide education and support as required on an individual basis and / or in a class setting including during the pre-operative period and training to self-manage ongoing consequences of treatment.
- Have daily contact with in-patients ensuring support for all aspects of the enhanced recovery pathway, with a particular focus on mobility and psychological support
- Call all patients within a week of discharge to offer support and advice in the immediate post-operative period, follow-up with further support as required on an individual basis and / or in a class setting.
Provide support, information and advice to patients, carers and other members of the multidisciplinary team on all aspects of the service.

Detailed job description and main responsibilities
The Clinical Nurse Specialist will:
- Put quality at the heart of practice by delivering evidence based individualised and personalised care through holistic needs assessment, planning and evaluation of care and all care interventions.
- Work at a highly specialised level, practicing autonomously as well as within a multi-disciplinary team and are self-directed.
- Actively integrate theory and practice.
- Act as a Key Worker for a clinical caseload, working to coordinate care and provide easy access for ongoing support for patients.
- Assess people’s information needs and provides relevant information to meet those needs.
- Enable people to access, identify, appraise and interpret information.
- Plan, prioritise and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, providing direct support to patients and clients.
- Lead the nursing contribution to service development, ensuring the pursuit of excellence in care.
- Use professional judgement in managing complex and unpredictable care events.
- Draw upon an appropriate range of multi-agency and inter-professional resources in their practice.
- Appropriately define boundaries in practice.
- Promote a high standard of nursing by initiating and co-ordinating the assessment, planning, delivery and evaluation of the holistic needs of patients / families through evidence-based practice following agreed policies, protocols and guidelines.
- Practice a high standard of communication including highly sensitive and complex information to develop and sustain partnership working with individuals, groups, communities and agencies. Utilise evidence-based information through a variety of communication media.
- Maintain adequate patient documentation to NMC requirements for all patients seen and advice given in any practice setting and contribute to clinical activity / data collection as required.
- Support patients in the pre and post-operative phase to be able to achieve a maximum state of health physically, psychologically and emotionally.
- Promotes self-management, undertaking risk stratification to determine those who can self-manage, following education, those who will need guided support to self-manage and those who will require on-going, face to face support.
