Job overview
The Children’s Rapid Response Service have an exciting new opportunity as it continues to expand the team. The Children’s Rapid Response Service works collaboratively with the Community Children’s Nursing & Palliative Care Team: It is the team’s primary aim to prevent and reduce hospital attendances, admissions and length of stay for children already known to the Community Children’s Nursing & Palliative Team.
We are looking for a Senior Children’s Rapid Response Nurse to manage and support a wide range of complex health and social care issues for patients who are acutely unwell and can be safely treated in their own homes. You will be overseeing the effective and efficient use of skills and resources to ensure the team and the service is organised to meet the needs of the children, young people and their families.
Full time/part time/flexible hours of working will be considered at interview.

The post holder will have an opportunity to further develop their clinical skills in various clinical conditions within a supportive environment which encourages learning.
The emphasis of this service is to improve the quality of life for children and families with life limiting, palliative or complex healthcare needs, offering them greater choice around their chosen place of care.

Detailed job description and main responsibilities
Professional
- Provide highly specialist clinical expertise as a required by the role to prevent hospital admissions or to expedite an early hospital discharge.
- Provide comprehensive evidenced based and skilled nursing care/intervention to children within their own home / family home/ other settings within specialist nursing services e.g. Special schools.
- Work with clinical leads to maintain and improve the quality of practice and service delivery.
- Identify, alongside the clinical lead, training requirements for CRRT.
- Responsible for accepting referrals, ensure assessment, planning and delivery of holistic care is at the highest standard using an agreed nursing model. Ensuring team discharge patient on completion of episode of care.
- Facilitation of difficult conversations around advance care planning and treatment. This would incorporate the philosophy of the child’s best interests whilst recognising the potentiality of deterioration may be lead to the dying phase. This would need to take into account language barriers, understanding and wishes
- Work within the community (e.g. within client’s own home) as a lone practitioner managing situations of risk with telephone support. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility, knowledge or skills.
- To provide a calm and supportive approach to the management and delivery of sensitive and complex information, providing both practical and emotional support, utilising flexibility and empathy in difficult situations.
- Use effective communication techniques communicate to the multidisciplinary team to ensure the child and family referred to Children’s Rapid Response receive the best possible care.
- Work in partnership with children, families and carers to provide optimum individual outcome acknowledging children and family’s personal preferences, spiritual and religious beliefs.
- To lead timely, effective symptom management, resulting from a comprehensive history taking, utilising independent non medical prescribing to ascertain a differential diagnosis, completing symptom flow charts to monitor the child’s changing condition/deterioration providing highly specialised care.
- Liaise and work in collaboration with acute services, primary healthcare team and other agencies when planning and implementing programmes of care facilitating early discharge and preventing admissions.
- Provide a flexible service to accommodate the changing needs of children and families.
- Recognise situations, which may be detrimental to the child’s physical and psychological well-being, and to deal with individual situations according to Trust and local agency policies.
- Undertake risk assessments prior to service delivery within the home setting
to ensure staff safety.
Management and Leadership Responsibilities
The post holder will be required to deputise in the absence of the Team Leader and undertake the responsibilities as outlined.
- Develop and empower all members of your team to perform to high standards and innovate.
- Ensure supportive staff management arrangements are in place and carry out personal development reviews for direct reports. Ensure all staff in your team/s have annual PDR’s resulting in specific objectives and effective personal development plans in line with the Knowledge and Skills Outline Framework.
- Develop staff knowledge and skills to promote equality and diversity and address inequalities both in employment and service delivery. Ensure specific equality objective are included in PDR’s.
- Develop a culture that ensure that the standards of Improving Working Lives and Investors in People are achieved and maintained for all staff and that staff’s perception about their working lives are measured and improvements made.
- Undertake effective day to day management of the CRRS, including performance management of individuals where necessary. Assisting caseload, holders to determine priorities of care, co-ordinating skill mix on an on-going basis.
- Is responsible for the supervision of the team, ensuring availability of service to meet both client and staff needs.
- Will undertake management responsibilities of the team including: staff rotas, cover for absence, sickness, and other absences. Input into recruitment and selection of staff, liaising with senior management where appropriate.
- Facilitate and ensure completion of staff Personal Development Reviews, monitoring attendance at mandatory and other education and training sessions.
- Act as a role model by demonstrating leadership and expertise and maintaining credibility across the organisation, wider health community and external agencies ensuring that positive reputation of the organisation is maintained.
- Facilitate and/or participate in the teaching and mentorship of staff, ensuring they have the knowledge and skills to fulfil their roles.
- Facilitate regular team meetings to ensure good communication within the team to foster, maintain and enhance effective working relationships within the team.
- Participate in research and audits relevant to the service and support both the service manager and clinical lead to conduct audit activity by continually monitoring standards of care, identifying risks, advancing knowledge within own specialist field, benchmarking and raising concerns to line manager.
- Assist Team Leader to appropriately manage the team resources including equipment and pay for the CCRT.
- Ensure that all team members engage in regular team reflection/clinical supervision.
- Participate in developing service policies, pathways, procedures and guidelines in partnership with relevant specialists, ensuring clinical governance is central to the function of the service, taking the lead with developing CRRS policies alongside the clinical lead with the aim to shorten hospital admission, attendances whilst offering greater choice to children and families around chosen place of care.
- Maintain systems and processes to promote a healthy and safe working environment ensuring compliance with legal and regulatory requirements maintaining accurate documentation and reporting any concerns to the line manager.
- Participate in the induction and orientation of new staff, ensuring mandatory training takes place according to organisational policy.
- Deal with informal complaints from service users and assisting with formal investigations as requested.
- Accurately record and maintain client records daily using electronic and paper systems e.g. RiO.
- Collect statistical information relating to client daily contacts e.g. response times and interventions, recording it on RiO/diary sheets in accordance with Trust and Local Authority requirements.
- Work with Service Manager to monitor activity, performance and quality of service delivery against contractual requirements.
- Responsible for Risk Management and Health and Safety of the CRRS.
- Ensure staff maintain trust standards in record keeping. Maintain clear and contemporaneous records. Ensure information is obtained and recorded before undertaking care.
Education and Development
- Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including participation in the appraisal process, supervision and maintain a professional portfolio.
- To identify and undertake specific training to maintain skills in line with NMC Scope of Practice and development needs of the service.
- Participate in the provision of in-service training for all grades of staff.
- Undertake teaching mentorship and evaluation of students both at the university
and during clinical placements providing specialist knowledge. - Maintain own professional development in line with post-registration on going education and practice (Prep).
