In Bristol, North Somerset and South Gloucestershire, the NHS, local government and the voluntary sector have been working in partnership for many years to improve care, provide more joined-up services, and to agree and plan for local people’s needs. Because of this, we received formal Integrated Care System (ICS) status in December 2020.
From 1 July 2022, our ICS became a statutory (legal) entity under the The Health and Care Act.
Known locally as the Healthier Together Partnership, our ICS includes councils, NHS hospitals, GP practices and community and mental health services. Together, we are working to improve health and wellbeing, reduce inequalities, and provide services for the one million people living in Bristol, North Somerset and South Gloucestershire.

Job overview
To provide comprehensive nursing / health assessments to determine the eligibility of clients for full NHS funded care. Ongoing case management / co-ordination for clients receiving full NHS continuing health care funding and NHS funded nursing care.
Must have a driving licence and be mobile for the post.

To work in a collaborative manner with, Health & Social care colleagues in the co-ordination and best interest of the client.
To work in collaboration with the private sector Care Homes to facilitate their staff in accessing appropriate education opportunities.
Maintain clear and concise and up to date records of care, in line with approved standards.
Proactively raise issues and provide solutions in partnership with other colleagues in relation to change management, working across agencies to enhance seamless care.
To process allocated applications for full NHS funded care in a concise and timely manner, ensuring a robust system is in place at all times.

You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

Knowledge, Training and Experience
Essential criteria
- Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
- Further training or significant experience in project management, or supporting workforce change management
- Experience and understanding of evaluating and measuring the impact of workforce/Learning/Leadership projects and programmes
- Experience in communications and complex stakeholder management
- Strong Microsoft office skills including Outlook, Excel, Word and powerpoint
- Experience of managing risks and issues, capturing actions and holding to account for delivery of agreed actions
- Experience of drafting reports and presentations

- Comprehensive knowledge of project management and/or workforce development/training
