We are therefore looking for an enthusiastic nurse to join our team as the Oncology Discharge Co-ordinator. Ideally, the postholder will have experience of working closely with patients who have complex health needs and/or housing and social placement issues. A good understanding of the NHS continuing care framework processes is an advantage although not vital as training will be provided. Preferably the successful applicant will have oncology nurse experience.
This post is intended to support the inpatient oncology service at UCLH. The overall aim of the post is to improve the quality of service our patients receive and to facilitate discharges that effectively meet the individual needs of our patients. To achieve this, the successful candidate will need to provide senior support to the multidisciplinary team to facilitate safe and timely discharges for patients back to the community, referring to hospitals, hospices, residential care or other preferred place of care.

This is an exciting post where no two days are the same. You will be expected to work independently within a supportive MDT. We are looking for somebody to complement the team who is enthusiastic and conscientious. We are also looking for someone who has an desire to teach and empower the team in completing discharge processes in an effective and timely way.
It is essential the successful applicant possess excellent verbal and written communication skills as they will be required to work closely with cancer patients and their carers. You will have great organising skills and the ability to prioritise your workload and to work to tight deadlines. The post holder will need to be able to use their own initiative and utilise problem solving skills. For the post holder to be successful they should be accurate and take pride in their work. The post holder need to collate, when requested, audit data and present in a format for distribution.
