Job overview
An exciting opportunity has arisen for the post holder to work within an established hospital and community cardiac rehabilitation service. The post holder will be expected to participate fully in the delivery of a comprehensive programme of Cardiac Rehabilitation sessions at hospital and local community sites.The successful applicant must be highly enthusiastic and motivated to deliver generic and exercise based cardiac rehabilitation with a strong knowledge base in health care and exercise based cardiac rehabilitation. The post holder will have proven experience within a health care setting delivering both individual and group exercise and educational sessions in a variety of formats, and be competent and confident in communicating at varied levels.
A demonstrable working knowledge of cardiac rehabilitation, exercise physiology, coronary heart disease and an up to date knowledge and application of relevant exercise based guidelines and frameworks /core components is essential. A minimum degree level education is required with previous NHS /exercise experience in the delivery of cardiac rehabilitation . Exercise instructor qualifications such as BACPR Phase iv is also required (or equivalent) .
Candidates will require access to transport for work purposes and may be required to attend home visits and sessions at local satellite centres (Ellesmere Port Hospital).
This post is part time - 25.5 hours per week.
Advert
- Deliver a wide range of exercise based cardiac rehabilitation sessions in line with BACPR and ACPICR standards. Deliver: circuit sessions, 1:1 sessions, strength training, low impact circuits, strength and balance exercise sessions, strength and conditioning sessions . Deliver exercise assessments and evaluation sessions for all the above;
- Supervise, and lead exercise-based cardiac rehabilitation sessions in line with current BACPR ACPICR/AHA and AACPVR guidelines;
- Assess the clinical suitability of patients attending for exercise/functional assessments;
- Prescribing exercise to patients with wide ranging pathophysiology and co-morbidity.
- Delivering of outpatient cardiac rehabilitation to include educational talks ( exercise and relaxation classes.
- Plan, implement and devise procedures/actions to monitor the effectiveness of safe exercise based cardiac rehabilitation.
- Ensuring that patients progress through the relevant stages of the programme at the correct time and that appropriate information and advice is provided at each stage.This will include patient monitoring and initiation of further referrals to specialist services (e.g. psychology, wellbeing, smoking cessation).
- Participate fully in MDT;
- Ensure all data is completed and entered on departmental database (CCAD national minimum dataset and hospital based systems).

Essential criteria
- Degree in exercise science, biology, nutrition or related discipline (minimum 2:1 classification).
- BACR Phase iv Instructor qualification

Knowledge and Expertise
Essential criteria
- Up to date knowledge of national and local strategies to reduce CHD (BACPR, AACPVR, NICE).
- Good level knowledge of existing exercise testing and prescription guidelines.
- Demonstrable and competent knowledge of exercise physiology and impact of varied clinical pathologies
- Experience of developing, leading and evaluating exercise based cardiac rehabilitation for specialist populations.
- Experience of working to time schedules
- Experience of planning long term exercise programmes and evaluating outcome
- Ability to deliver educational talks and leading group exercise sessions.
- Ability to assess clinical information and structure patient care accordingly.
- Ability to support senior colleagues in designing and implementing training programmes and research & audit activities
