Urgent Community Response Triage Sister/Charge Nurse Band 6
Opportunities for various shift patterns - Urgent Community Response Team (Integrated Single Point of Access (ISPA))
An exciting opportunity has arisen to join our innovative Urgent Community Response Team as part of the wider Out of Hospital services within Coventry. The role will require you to use your sound clinical knowledge and risk assessment skills to medically triage patients presenting with acute health needs via the telephone and available telehealth systems, to effectively coordinate services and ensure the most appropriate response is mobilised to safely meet patient need.
As the role progresses, it will be supported by the use of a clinical triage tool which follows nationally accredited algorithms to ensure a safe and consistent approach to triage with access to senior clinician support as required.

You will be working alongside a highly skilled workforce including Consultant Nurses, Advanced and Clinical Practitioners, Reablement Sisters, Occupational and Physiotherapists and Healthcare Support Workers and liaising closely with wider members of the MDT including GPs and Care Navigators.
As a valued team member your development will be supported through a robust induction programme, a value-based appraisal, one to one meeting’s, regular in-service training and clinical and managerial supervision. You will work out of a multi-professional base so there is opportunity for joint working and collaborative learning across a range of Health Professionals who are supporting and delivering high standards of clinical care. Training opportunities are supported by our Practice Based Facilitators and Talent for Care team and is actively encouraged by Team Leads.
Our working hours are 8am to 8pm over a 7-day flexible service with no night shifts. Applications for flexible hours including long days will be considered on an individual basis. Car owner/driver is essential.
Although this role is predominately a non-patient facing role, there maybe occasions when you will be expected to support clinical patient facing tasks with a safe scope of practice.

Adult Physical Health
Coventry and Warwickshire Partnership NHS Trust (CWPT) has taken the decision to no longer continue to provide Adult Physical Community Services, as there are more suitable providers within the wider NHS local system.
Following a review of suitable providers to take forward the contract, the Coventry and Warwickshire Integrated Care Board (ICB) has confirmed its intention to award the contract for the provision of Adult Physical Community Services to University Hospitals Coventry & Warwickshire NHS Trust (UHCW). This change is anticipated to take effect from 1st July 2024.
Your application to work in Adult Physical Community Services will not be adversely impacted by the change in provider. Should you commence employment prior to the transfer, your contract of employment (that is your terms and conditions of service as at the point of transfer) will automatically transfer to the new organisation, under the "Transfer of Undertakings (Protection of Employment) Regulations 2006" (TUPE) as amended by the "Collective Redundancies and Transfer of Undertakings (Protection of Employment) (Amendment) Regulations 2014".
Should you commence employment after the transfer, your contract of employment will be issued by the new provider of Adult Physical Community Services.
