With an ageing population and pressures on funding, it’s getting harder for GP practices to ensure everyone in the community can access the same level of care. Yet, our diverse communities harbour a host of health problems from smoking-related issues to obesity, so making sure that residents from all walks of life can get the treatment they need when they need it most is paramount. 

That’s where Primary Care Doncaster (PCD) comes in. We work for your local GP practices, offering services that make gaining access to healthcare more convenient and simple for everyone. To this end, provision of additional access to appointments, bringing medical advice and treatments directly to the community via Vinnie Van-Cough our Health Van is a key part of our role.

Behind the scenes, we give GP practice teams the tools and training to transform and come up with innovative, new ways of delivering patient care, as well as represent the general practice community through ‘one voice’ with other local health and care partners in the system

We aim to become indispensable to GPs by helping to shoulder the burden of provision of care and release the pressure that is currently being put on practice teams.

We help our local primary care networks (groups of GP practices serving each neighbourhood) to grow and work together in a way that is both holistic and integrated so that any inequalities can be ironed out and every patient in the Doncaster area can benefit from the very best proactive treatment and care.

We are recruiting Care Coordinators to join our team!

This role will be working in the North Primary Care Network.

This is an exciting opportunity in which you will gain experience and make a real impact. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within the Primary Care Networks.

The ideal candidate should either be educated to NVQ Level 3 or have equivalent knowledge and experience of office procedures; including a wide range of administrative systems and software. Previous experience of Primary Care or the NHS would be an advantage, however is not essential.

The successful candidate will need to be flexible in their approach to work as the role will be working primarily across the Primary Care Networks, within various practices in Doncaster to undertake duties including;

Help and support the multi-disciplinary team (MDT) meetings and patient case management

Support the MDTs to deliver effective and co-ordinated care for vulnerable and frail adults; including those at high risk of hospital admission

Help people manage needs, e.g. answering queries, managing appointments

Assist people to self-manage their health and wellbeing

Support patients to utilise decision aids in preparation for shared decision-making conversations

Detailed job description and main responsibilities

To work closely with teams to help and support the multi-disciplinary team (MDT) meetings and the ongoing patient case management. This will involve linking with GP surgeries and a range of Community Health Services.

To support the MDTs within the Primary Care Network (PCN) to deliver effective, co-ordinated care for vulnerable and frail adults, particularly those at high risk of a hospital emergency admission, emergency department (ED) attendances or out of hours care.
