Job overview
Operating within clearly defined limits of the policies, procedures, protocols and guidelines of the service the Patient Pathway Navigator will be responsible for the validation process of the 18 week referral to treatment (RTT) target within the hospital. The post holder will track the progress of patients through care pathways and core services to ensure that patient treatment progresses smoothly and in a timely manner.
To develop, introduce and maintain systems and processes to guarantee an efficient and effective service that supports the delivery of activity and performance targets. He/she will be required to liaise closely and work collaboratively with clinical, nursing and administrative teams within the hospital to ensure delivery of the RTT target.
To evaluate the quality of information recorded by clerical and admin staff within the hospital in relation to Trust and NHS standards. Collecting relevant data and participating in audits as required.
Support and work closely with the Information & Development Manager deputising for them and attending the Trust Information and Data Quality meetings as required

- To coordinate the achievement of the RTT patient pathway in line with Trust and National targets.
- To use IT systems to track patients along their pathway from referral to first definitive treatment and escalate to the line manager any variances which may impact on the achievement of the RTT target.
- To ensure accurate, timely validation of RTT closed and open pathway data is undertaken on a daily basis escalating any potential issues to the line manager.
- Identify any pathway restrictions within RTT pathway escalating any issues to the line manager and work with multi-disciplinary teams to resolve identified issues.
- To produce training tools to facilitate on going awareness and education on RTT pathways for administrative, nursing, and clinical teams within the hospital.
- Monitoring of RTT waiting times producing waiting time reports and pathway achievement reports to be tables at activity meetings on a weekly basis.
- Ensure accurate recording and linking of patient pathways using appropriate PAS functions.
- Identification of any patients within the system who may have the potential to breach the RTT targets escalating this through the management structure and the multi-disciplinary teams taking responsibility to review pathway processes.
- To locate and retrieve case notes for review from all areas of the hospital as required.
- To assist in the training/induction on the RTT patient pathway for new recruits to the hospital.
