Job overview
This is an exciting opportunity to be part of a small, specialist service that works under the For Me Provider Collaborative (Thames Valley and Wessex adult secure PC).
FINDS, the Forensic Intellectual and Neurodevelopmental Service, works alongside other services who are supporting individuals with a current forensic need and a diagnosis of either/or learning disabilities, Autism and/ or acquired brain injury by offering consultancy, training, and advice, as well as the gatekeeping into secure inpatient services within the For Me footprint.
In our small but experienced team, you will take on a specialist consultancy role of specialist health professionals, including nursing, psychology and psychiatry. We'll need you to ensure that that the teams you are offering consultancy for work within the principles of the Mental Health Act, the Mental Capacity Act and the NHSE secure service specifications to ensure that those referred are offered the most appropriate pathway outcomes.

In this role, you will be:
- Working within a multidisciplinary team to complete robust access assessments for secure care for this population group within the Thames Valley & Wessex region.
- Monitoring the individual as they move through secure services and feed into CTRs and CPA meetings, to ensuring assessed needs are met in the most appropriate way and that discussions about discharge are part of pathway planning.
- Ensuring barriers to discharge are expediently addressed.
- Working with different services and agencies to promote and encourage communication and sharing of information, relevant to the support needs and risks of the service users.
- Maintaining an overview of, and relationships with, community support provision within the Thames Valley & Wessex region. In particular specialist LD/ASD/ABI & MH provision and other services within the wider community.
- Using professional knowledge, skills and evidenced based practice to provide upskilling, consultation and support to mainstream teams in the community during transition of the patient to the community.

Detailed job description and main responsibilities
Within consultancy, you will deliver training and consultancy, liaise with various services across the footprint, including the case managers within secure, the Integrated Care Systems, secure hospitals, community mental health and learning disability community teams, social care, prisons and other nonsecure hospitals.
The role of FINDS is to enhance the skills and knowledge of providers to reduce the need for secure hospital, follow the person through their stay in secure offering support around reasonable adjustments if required and to offer teams supporting the person on discharge training and understanding around the persons needs for a 6 month period to reduce readmission to secure hospital.
The role is Monday to Friday, but consolidated hours or job share may be considered. With discussion, this role could be a developmental role for those without relevant band 6 experience.
The successful candidate will be able to work flexibly across their home and base, as long as they are available for any in person contacts required across the For Me footprint. There are various internal and external learning opportunities available around the specialist needs that FINDS support and opportunities to access developmental opportunities within the team and the provider collaborative to ensure that the services within For Me are delivering safe and effective services.

Essential criteria
- Assessment skills and experience of assessing individuals that have complex MH/LD and/or forensic needs.
- To understand and have experience of risk management in this client group.
- Experience of discharge planning and supporting transitions.

- Understand the legal frameworks around mental health and learning disability.
Desirable criteria
- To have awareness of National and local issues concerning this client group (Transforming Care, C(e)TRs), including NICE guidelines.
- Knowledge of Autism and ABI, or willingness to upskill in these areas.
- Awareness of the Criminal Justice Pathway and the agencies that work alongside it.

- Experience at band 6 or equivalent, with experience of management of community or in-patient environment/team.
- Experience of managing high risk, complex and difficult clinical situations and cases.
- Experience in providing formal/informal teaching.

- Wide ranging ability to undertake supervision of registered and non-registered staff.
