Swydd-ddisgrifiad a phrif gyfrifoldebau manwl

1. Clinical:
• To provide comprehensive and holistic assessments of clients, their families and/or carers that are referred to the Adult Service that synthesises information from a variety of sources.

• To hold a caseload that covers clients from different parts of the Adult Service.

• To be responsible for implementing, developing and evaluating systemic interventions for individuals and their families and/or carers. This includes signposting families and/or carers to community resources as well as supporting the team to develop a family-focused practice.

• To provide and help facilitate evidence based therapeutic groups for clients and their families and/or carers, both separately and conjointly. These include skills workshops and support groups for families, partners and those in the network surrounding the client, as well as community meetings on the inpatient ward and day programme.

• To help teams within the service to formulate and implement care plans for the treatment and management of the client based on using a systemic framework. This includes providing input in team meetings and ward rounds.

• To contribute directly and indirectly to a systemic framework of understanding and care to the benefit of all clients of the service, across all settings and agencies serving the client group.

• To undertake risk assessment and risk management for individual clients and their families and/or carers, and advice other professions in this area where appropriate

• To communicate assessment, formulation and treatment plans to individual clients and their families and/or carers in a collaborative and sensitive manner

• To monitor the progress of clients and be able to review the need to change the care plan where necessary

• To ensure that all services provided are acceptable and accessible to the diverse communities which make up all the boroughs in CNWL. At all times to work in ways that are sensitive to and appropriate for the needs of families from a wide range of racial, cultural and religious backgrounds, and to be aware and sensitive to the different contexts that shape different families experiences, beliefs and perceptions. This will include working with interpreters.

2. Education and Development
• To receive regular clinical professional supervision from a senior family therapist and, where appropriate, other senior professional colleagues.
• To complete and stay up to date with the CNWL mandatory training programme annually and within the required timescales.
• To attend Staff Group with other colleagues in the service
• To attend the Family Therapy Forum for peer supervision, case discussion and opportunity for overall systemic learning and development

3. Research, Audit, Service Development and Evaluation
• To contribute to the development, evaluation and monitoring of the team’s operational policies and services that draw on research, service valuation and audit
• To advise both service and professional management on those aspects of the service where clinical and/or organisational matters need addressing
• To utilise theories and research to support evidence-based practice in clinical and team work
• To be involved in research where appropriate.
• To contribute to the development and maintenance of the highest professional standards of practice through active participation in internal and external CPD training
• To contribute to the development of best practice in family therapy across the service

4. Administration and Clinical Recording
• To enter required data into the Trust’s patient administration systems and to make use of data bases as directed by managers in the course of clinical duties
• To use computer and computer software to prepare reports, presentations and documents relevant to working duties
• To undertake administrative tasks in line with service requirements, and make use of the administrative support where appropriate

5. Standard
• Multi-agency Meetings
The post holder will prioritise attendance at relevant multi-agency planning meetings such as ward rounds, patient reviews, professionals meetings, CPAs, as well as Child In Need and Child Protection conferences where applicable

• Policy and Procedure
The post holder is expected to comply with all relevant Trust policies, procedures and guidelines, including those relating to Equal Opportunities, Health and Safety, Confidentiality of Information, Sickness Absence and clinical governance (including research governance).

• Professional Standards and Performance Review
The post holder will fully participate in the Trust’s Appraisal / Supervision Programme and work with their line manager to ensure that they have a clear job plan and objectives
