This is an exciting opportunity for an ambitious leader to be at the heart of the development and delivery of initiatives for Primary and Community Care, in line with SWL ICB and Croydon Place based priorities. The post holder will ensure that plans bring together primary and community care to ensure the progressive improvement in health outcomes for local people, enhanced productivity, efficiency and best value in models of service delivery, whilst promoting innovation.
The successful candidate will have experience at a senior level with regards to transforming and integrating health and care services, achieving change through partnership and leading a team. They will have a good understanding of NHS and care services, commissioning, programme management, community development and the prevention agenda.

Key duties of the role
· Lead the integration of primary and community care in Croydon Place (as part of South West London ICB), ensuring alignment with the local Health and Care plan and the One Croydon Alliance priorities.
· Lead the delivery of the wider prevention, proactive care and access improvement objectives as set out in the SWL primary care Strategy.
· Ensure good local and regional governance for decision making and for the development and implementation of delivery plans.
Be a key member of the Croydon Place leadership community, deputising for the Director of Transformation and Commissioning; working closely with the local NHS Trusts, Council, primary care, community health providers and the VCSE sector; as well as working closely with colleagues across Southwest London ICS.

NHS South West London Integrated Care Board (ICB), as part of South West London Integrated Care System (ICS), is a partnership of organisations that come together to plan and deliver joined up health and care services to improve the lives of people in our six boroughs: Croydon, Merton, Kingston, Richmond, Sutton and Wandsworth.
Each ICS consists of two statutory elements:
- an Integrated Care Board, bringing the NHS together with its partners locally to improve health and care services
- an Integrated Care Partnership (ICP): the broad alliance of organisations and representatives concerned with improving the care, health and wellbeing of the population, jointly convened by the ICB and local authorities in the area.
ICBs are statutory NHS bodies responsible for planning and allocating resources to meet the four core purposes of integrated care systems (ICSs):
- to improve outcomes in population health and healthcare;
- tackle inequalities in outcomes, experience and access;
- enhance productivity and value for money and;
- help the NHS support broader social and economic development.
NHS South West London Integrated Care Board decides how the South West London NHS budget is spent and develops plans to improve people's health, deliver higher quality care, and better value for money.

. Duties and Responsibilities
• Ensure that primary and community care developments are aligned with wider
system transformation programmes.
• Develop and implement new models of care in primary care that complement the
approach to new ways of working in the emergent integrated care systems (ICS)
models as appropriate to each area ensuring that they are grounded in strong and
resilient primary care services.
• Ensure that Croydon Place apply evidence based, effective and legally compliant
procurement and contract management practice across the primary and community
care commissioning portfolio.
• Maintain and strengthen strong collaborative working relationships with all primary
care and out of hospital providers and with respective professional bodies.
• Lead the Croydon planning processes relevant to delivery of enhanced primary care
and community services in accordance with SWL and place initiatives
• Translate primary care and community services guidance and its relevance to place
transformation.
• Re-design patient pathways and re-configure services to support the provision of
services in the most appropriate settings for patients and those accessing services,
including managing and where necessary reconfiguring resources to support this.
• Design and deliver service redesign business cases, PIDs/service
specifications/action plans and/or proposals, leading these to delivery.
• Manage and lead a programme of transformation developments in conjunction with
clinical leads to improve the health outcomes in Croydon. This will involve proposing
and agreeing developments via relevant Place Committees for Croydon.
• Support the development of primary care networks and working with the Croydon GP
Collaborative, ensuring at scale services are robust and providing sufficient support
to general practice.

• Guide, mentor and support clinicians and other key stakeholders in the analysis and
interpretation of complex data to diagnose problems and make recommendations
based on options, monitor implementation and evaluate effectiveness.
• Present complex and sensitive information to open forums, conferences, regional
meetings regarding service delivery and redesign. To be able to use developed
formal and informal communication skills to convey, sometimes contentious
information in an appropriate manner.
• Ensure patient representation and involvement is fully accountable and integrated
into local projects.
• Ensure that all services adhere to and achieve standards and targets outlined by the
SWL ICB Commissioning Plan within identified timescales and that reporting systems
are robust.
• To provide visible and effective leadership to the Primary Care and Community
Development and Delivery Team, including supporting the development of team
members
• Work closely with One Croydon Alliance partners, including Croydon Council, Croydon
Health Services, South London and Maudsley NHS, Croydon GP Collaborative and
Croydon based Voluntary Sector, social enterprise and faith organisations

Education / Qualifications
Essential criteria
- Educated to masters level or equivalent level of experience of working at a senior level in specialist area
- Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master’s level equivalent
- Evidence of continued professional development

Essential criteria
- In-depth professional knowledge in a number of disciplines such as financial management, performance management, information systems, staff management acquired through raining and experience over extended period
- Previously responsible for a budget, involved in budget setting and working knowledge of financial processes
- Demonstrated experience in a Healthcare environment
- Extensive experience of setting up and implementing internal processes and procedures
- Experience of managing and motivating a team and reviewing performance of the individuals
Skills and Abilities
Essential criteria
- Highly developed communication skills with the ability to communicate and present on highly complex, sensitive and/or contentious matters and difficult situations
- Ability to persuade board and senior managers of the respective merits of different options, innovation and new market opportunities
- Ability to negotiate on difficult and very complex and detailed issues
- Intermediate (or advanced) Keyboard skill – ability to use Microsoft Office package at intermediate (or advanced) level
- Ability to analyse numerical and written data, assess options and draw appropriate conclusions
- Ability to develop, maintain and monitor information systems to support innovation initiatives
- Ability to make decisions autonomously and on difficult and contentious issues where there may be a number of courses of action
