We have an exciting opportunity for an Advanced Clinical Practitioner to join our regional tertiary haematology unit here at Blackpool Teaching Hospital. The ACP team, led by a Nurse Consultant, is part of the large MDT providing highly specialised care to haematology patients from across Lancashire and South Cumbria. The successful candidate will work alongside colleagues to provide many services - procedures, clinics, inpatient/outpatient management, service development and more, to support delivery of a high quality effective service.

The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care demonstrated through the advanced competency framework provided through the MSc programme. S/he will provide advanced clinical skills and advice demonstrating a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of the client group. S/he will monitor and lead improvements to standards of care and the adherence to Haematology and Cancer Guidelines through supervision of practice, clinical audit, evidence based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership. S/he will lead the implementation of relevant research into practice contributing to the further development of the evidence base through research and audit. S/he will contribute to clinical governance within the service Advanced Practitioner Job Description by leading audit and research as part of the MDT within the area of expertise. The successful candidate will work in collaboration with medical, nursing and AHP colleagues.

Main Duties
1. Develop and maintain communication with people about complex issues and/or in difficult situations.
2. Be proactive in developing own professional practice, demonstrating evidence of increasing autonomy, clinical judgement and decision-making skills and contribute to the development of others.
3. Promote, monitor and maintain best practice in health, safety and security.
4. To act as the day-today expert operational lead for Haematology, working
autonomously and in collaboration with multi-disciplinary professionals both internal and external to the trust.
5. To provide clinical leadership and work alongside the Haematology Consultants to develop evidence-based practice and maintain clinical excellence, which will promote the delivery of quality care development.
6. To act as an expert practitioner demonstrating and applying advanced skills and knowledge in the area of Haematology and provide expert advice, including producing guidelines, and inputting into future developments, service planning and provision.
7. Assess the need for and order investigations within scope of practice to meet National Guidelines.
8. To develop nurse-led clinics to ensure Haematology patients receive appropriate treatment plans in line with national guidance.
9. Support equality and value diversity.
10. Plan, develop and implement approaches to promote health and wellbeing and prevent adverse effects on health and wellbeing.
11. Address complex health and wellbeing needs and develop, monitor and review care plans to meet those needs.
12. Assess physiological and psychological functioning when there are complex and/or undifferentiated abnormalities, diseases and disorders and develop monitor and review related treatment plans.
13. Having a visible presence within the key points of entry across the Trust to prevent non essential inpatient admissions and ensure timely discharges by the appropriate use of existing and the development of community services, enhancing patient experience
14. To lead on future developments within Ambulatory and Home Care for
Haematology Patients.
15. Gather, analyse and report a wide range of data and information.
16. Demonstrate a high level of complex decision making in relation to the
assessment, planning, intervention and evaluation of patient care.
