We are looking for an enthusiastic, motivated Nurse/Therapist, who demonstrates good interpersonal skills and is willing to work flexibly within the Integrated Discharge clinical team. Our aim is to support with the holistic assessment of patients in a hospital environment and create a personalised plan of care for all patients once assessed and to provide expert clinical intervention by promoting timely and appropriate decision making and assessments for patients

Assist the Discharge Coordinator with setting standards of care
Responsible for managing staff within the Integrated Discharge Team.
Responsible for ensuring excellent clinical standards at all times / leading by example
To provide and co-ordinate clinical care and support staff in decision making
To assist the managers in dealing with staffing issues across the team, providing senior nurse support and guidance
To act as a resource to facilitate a whole system approach to the transfer/discharge process and promote a positive experience for the patient
To lead in clinical decision making especially with regard to admission avoidance within the Emergency department
To holistically assess patients in a hospital environment
Create a personalised plan of care for all patients once assessed.
The service is operational 8am-6.30pm Monday to Friday and 8am -4pm Saturday and Sunday. Shifts covering all of these hours are required for this role.

Detailed job description and main responsibilities
Provide expert clinical intervention by promoting timely and appropriate decision making and assessments for patients
Participate in the writing of reports in an accurate thorough way using IT
The post will involve liaison with other intermediate care services within the Medway area, primarily the community rehab teams but also our Council colleagues
Work with multi-disciplinary team to facilitate timely and appropriate clinical management of patients
Develop and maintain good working relationships with inpatient and community services within the organisation and with external bodies
To work as a key member of the transfer/discharge process within Medway, ensuring effective communication and developing sound relationships with colleagues, health and social professionals from within and external to Medway Community Healthcare, as well as colleagues in the voluntary and private sector
Provide clinical leadership and act as role model for all staff
Registered nurse or Therapist with evidenced relevant experience such as previous A&E, community or Nurse in charge experience
Extended nursing skills to include:-
· Registered nurse 1st level/Therapist or educated to degree level
· Evidence of continuous development
· Obtain and interpret patients’ observations
· Know principles of infection control
· Sound clinical knowledge and skills in the support of immediate management of patients requiring clinical intervention
· Evidence experience in identifying and managing unwell patients
· Experience of multi-disciplinary working
· DOLS
· Able to work flexibly and cooperatively as well as autonomously

· Confident in the MCA 2005

Ad-hoc management of in-patient areas and Medway Hospital site to ensure patient and staff safety
To assess practice and standards on the wards and in the community of Infection Control Precautions and to ensure the organisations policy is applied at all times

Co-ordinate the transfer of patients, including those with complex health and social care needs, between the Medway foundation trust and Medway Community Healthcare.
To assist the Clinical Lead and Discharge Coordinator to manage staffing issues across the team providing senior nurse support and guidance. Deputise for the band 7’s.
Provide professional leadership developing teaching and supervising staff, patients and their families to raise understanding, standards of professionalism and contribute to the development of nursing.
To carry out staff PDR’s, 1-1’s, informal sickness reviews up to and including stage 1 and manage a small team of band 4 and 5 staff.
Good computer skills in Word, Excel, Outlook and other software packages.
Patient moving and handling, including transfers patient from bed to chair and similar in order to support ward staff.
Be able to travel to dispersed sites in a timely manner if required.
To provide expert clinical knowledge and skills in the support of the immediate management of patients requiring clinical intervention.
Offer guidance to ensure patients have an appropriate management plan.
Provide professional leadership and act as a role model advising, mentoring, guiding and supporting staff to understand and contribute to clinical practise out of hours.

Encourage, actively promote and participate in if appropriate, the use of MDT’s for managing the transfer of patients with complex needs.
Work with, support and educate the hospital staff to ensure that patients are transferred in a safe, appropriate and timely manner/ complex discharge process.
Communicate complex condition related information from assessment to patients, carers, families and members of the multi-disciplinary team/other professions

To demonstrate empathy with patients, carers, families and colleagues, ensuring that effective communication is achieved, particularly where barriers to understanding exist.

The post holder is required to maintain confidentiality of information regarding patients, staff and other health service business in accordance with the Caldecott guidelines, Data Protection act and Children’s Act and other relevant legislation as appropriate
Information Governance policies
To maintain accurate patient and staff records and ensure systems, both electronic and paper based are adhered to within organisational policy
Ensure appropriate data and information is collected, collated and communicated to appropriate personnel in Medway Community Healthcare and the Strategic Health Authority. This includes current bed state situation, weekly delayed discharges and maintenance of the rehab list for community hospital beds
Assists with and facilitate data collection programmes such as clinical audit by the use of agreed tools
Participate and implement organisation / hospital / ward audits and surveys

Is competent with current emergency resuscitation techniques, fire procedures and emergency plan both in theory and practice and is expected to take a key role in these emergencies.
