Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Job overview
An excellent opportunity has arisen for a Highly Specialist Physiotherapist with proven clinical skills and a passion for caring for people who require rehabilitation within the setting of respiratory needs and ventilation weaning, to join the rehabilitation physiotherapy team at Guy’s and St. Thomas’ Hospital. The team consists of a respiratory band 7 as well as rotational staff and is supported by respiratory and rehabilitation clinical leads. This is a permanent role.
The post holder will be a key member of our multi-disciplinary team delivering high quality rehabilitation for those transferred to this regional weaning unit and respiratory centre.

We are looking for a dynamic, skilled clinician and leader who will provide clinical assessment, rehabilitation treatment and management of patients with highly complex presentations including those on ventilation. They will need to demonstrate understanding of the impact of neuromuscular conditions and other long term conditions on the ability to rehabilitate and also respiratory care. They will need to take part in airway clearance as well as have an understanding of weaning. As a band 7 service lead, they will be committed to training , supervising and supporting the career development of junior colleagues, students and delivering the team management. They will demonstrate drive and enthusiasm to lead and contribute to service improvement initiatives through project work.
The post holder will have had experience at a senior level or can demonstrate they have developed rehabilitation clinical skills. They will have the opportunity to further develop their clinical and leadership skills, supported by a Clinical Specialist Physiotherapist, Deputy and Clinical Lead Rehabilitation Physiotherapist and as a wider member of the B7/8 rehab team.
The physiotherapy service has a high profile within the Lane Fox Unit and currently provides a 7-day service, reflecting the physiotherapy department’s commitment to meeting patient needs and delivering excellence. It is essential the post holder be able to contribute fully to 7-day working.

Detailed job description and main responsibilities
- To take a lead role in the advanced assessment and rehabilitation treatment and management of patients with conditions that have led to a long critical care stay and have transferred to Lane Fox weaning unit. This would include neuromuscular conditions, chronic lung conditions, and neurological conditions such as spinal cord injury
- Understand pathophysiological presentations and impact on likely rehabilitation outcomes
- Have a good understanding of respiratory assessment and be able to deliver treatment as part of the patients intervention including airway clearance
- Participate in complex MDT decisions, inputting into a range of issues including the potential for functional gains
- To maintain associated records, as an autonomous practitioner.
- To take a lead role in supervision, training and development of physiotherapists and students and MDT staff in the physiotherapeutic care of patients with long term respiratory complications and rehabilitation needs
- To provide skilled team and service leadership and be responsible for operational management, team development, implementation of policy and policy changes in own clinical area, and to set and monitor standards of practice.
- To supervise, initiate and participate in evidence-based projects, including recommendations for change in practice and to lead in departmental audit, quality improvement and research in their clinical area.

- Broad range of experience at Band 6 level such as critical care, surgery, neurology/rehabilitation and elderly care
- Specialist experience in the management of rehab needs and ventilation weaning

Desirable criteria
