Job overview
We have an exciting opportunity for an enthusiastic Individual to join our Specialist Haematology Nursing Team within Northumbria Healthcare NHS Foundation Trust. We are looking for a highly motivated nurse specialist to work within our dynamic well-established team. The successful candidate will be working alongside the senior Haematology Nurse specialists, ACP's and Consultant Haematology team providing a first-class service for patients with malignant and non-malignant Haematological diagnosis.
The role encompasses ensuring the delivery of a seamless pathway of care from diagnosis to treatment, pre-habilitation, and, where necessary, to palliative care. Acting as Key Worker to assess and support the holistic needs of patients, relatives/carers whilst working within the specialist multidisciplinary team to deliver evidence based care. This is a high profile exciting role and provides an excellent opportunity to transfer existing skills, knowledge and experience.

The successful candidate will work within the Haematology Team based at the Wansbeck Hospital, however, will be required to support other hospital sites within the Organisation. The post holder will ensure patients have access to specialised services and will provide specialist advice and support to this group of patients who have malignant and non-malignant Haematological Disease and complex care needs. Working alongside the Clinical Haematology Nurse specialists, ACP and Consultant team, you would be responsible for Planning, Implementing and Reviewing patient care pathways. You would be required to work within the Out-patient setting acting as a Key Worker to assess and support the holistic needs of patients, relatives/carers whilst working within the specialist multidisciplinary team to deliver evidence based care. The role also entails the running of Nurse Led and clinics ensuring relevant treatments are prescribed and distributed safely and on time as well as monitoring side effects and provision of supportive therapy.

Detailed job description and main responsibilities
- To act as the patients’ key worker, developing a professional relationship and to provide support at diagnosis through to treatment and beyond to survivorship or end-of-life.
- To provide professional advice on Haematological issues and be a resource of clinical expertise for other healthcare professionals within the Trust.
- To co-ordinate the patient's care pathway and promote personalised care.
- To contribute to service development in regard to the Haematology nursing service.
- To liaise with Multi-disciplinary colleagues to ensure best practice and safe and high quality care is provided.
