DYNAMIC SUPPORT KEYWORKERS – Liverpool (2 posts)
Supporting Children and Young People (up to 25 years old) with Autism and/or Learning Disabilities.
We are seeking to recruit individuals with a passion for helping young people and their families to get the right support from Health, Social Care, Education and other services.
You will join an established team of Dynamic Support Keyworkers in Liverpool working across a wide range of settings to coordinate and provide challenge to the system, thereby ensuring that young people and their families can access the right support to enable them to achieve better outcomes and remain safe in the community.
Dynamic Support Keyworkers work with young people with Autism and/or Learning Disabilities who have multiple vulnerabilities and are at risk of
- admission to mental health wards
- family breakdown
- sexual or criminal exploitation

Dynamic support key workers understand that the parents/carers are the experts in the child or young person’s care needs – they are there to support and empower families, they do this by.
· Being the main point of contact for the family and the involved professionals across the agencies/organisations
· Having regular contact with the family; listen to, support and work in partnership with the family, sharing information and following up on plans agreed
· Providing of information and advice, helping families to build their own confidence and be in control of the choices open to them
Should a child or young person be at risk of admission to a Mental Health Setting, the dynamic support key workers will work closely with the family and alongside other agencies involved to try and prevent this happening or will remain involved to try to make the hospital stay as short as possible and to facilitate a smooth discharge.
The dynamic support key workers have a significant role in supporting with the Care, Education and Treatment Review (CETR) process.

We are particularly keen currently to add skills and knowledge of Adult services, processes and legislation to our team in order to better meet the needs of young people aged 18 – 25 years.
Detailed job description and main responsibilities
Key responsibilities
- Develop and maintain a strong, positive relationship with children and young people with a learning disability, autism or both and to ensure that they and their families get the right support at the right time
- Provide support to access the right help at the right time across complex systems particularly the transition between children and adult’s services
- Build relationships and support coordination across the system, including Health, Education and Social Care
- Hold services across the system to account where action plans are in place or actions have been agreed
- Ensure transitions between placements, settings and services are well managed
- Keyworkers will be trained to apply the CNEST Risk Rating Tool and will work with other professionals across the system to identify children and young people who meet the criteria and would benefit from oversight via the Tracker meetings
- To facilitate, participate in and follow up actions from Care Education and Treatment Review (CETR) meetings.
- Champion Autism, raise awareness and promote strategies, increasing understanding.
- Troubleshoot and challenge on behalf of the child/young person and their family.
- Produce case studies, evidence outcomes and contribute to collection of data and feedback.
