As part of community mental health transformation we are seeking to develop the community rehabilitation pathway across Gloucestershire Health and Care NHS Foundation Trust with a particular focus on individuals with treatment resistant psychosis and frequent admissions or long stays in inpatient units locally, and out of county, as well as individuals with 24 hour support whose placements are at risk of breaking down.
This newly established team will seek to build links between internal services, VCSE (voluntary, community or social enterprise) organisations and existing/new care providers to ensure a multi-agency, collaborative and recovery-oriented approach to complex rehabilitation. This will include creating a shared ethos and goals and a sense of hope and optimism to deliver individualised and person-centred care to promote independence and integration within the local community.

As Mental Health Rehabilitation Co-ordinator the main duties will be:
- To support the multi-disciplinary team by determining recovery orientated goal plans including identifying recovery focussed activities and fostering a culture of hope
- Develop, implement and evaluate care plans in conjunction with service users, carers and other professionals which reflect individual needs
- Provide assessment and implementation of evidence-based interventions for service users who are experiencing common, acute mental health and longer-term recovery needs
- To work alongside a number of service users on a one to one or group basis, potentially within the individuals own home, or support junior staff, providers and VCSE organisations to carry out this work effectively

The post holder will be a senior member of the team working with service users across the Strategic Service Units who have a common and severe mental health. The purpose of this role is to:
- Provide assessment and implementation of evidence-based interventions for service users who are experiencing common, acute mental health and longer-term recovery needs.
- Optimise the service users’ ability to remain in the community and / or be cared for in the least restrictive environment in line with the principles or recovering and social inclusion and the 10 essential capabilities.
- Support clinical governance initiatives at ward / hospital level e.g. Essence of Care Clinical Audit.
- Provide supervision and guidance to junior staff and oversee the direct work being undertaken by the team

- Accredited post registration training/qualification (equivalent to Masters level) relevant to people with mental health difficulties

- Extensive experience of first line assessments including the assessment of risk
