Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.
In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. Job overview
The Health Inclusion Team (HIT) works across Lambeth, Southwark, and Lewisham to provide a multidisciplinary service for clients who are marginalized and have difficulty accessing primary and secondary care services (e.g. refugees and asylum seekers, homeless clients, and clients with addictions). The nurses have a broad range of advanced clinical skills and work autonomously in clinics in NHS/ non-NHS sites, so have to be competent and safe in their clinical decision-making. We currently have 1 Band 6 Community Nurse Specialist posts covering adults in the core initial Asylum accommodation service. The team is a multidisciplinary team offering core health assessments, minor aliment treatments, responding to immediate needs, and linking the residents into primary and secondary care. As a member of the team, you will be delivering a service across the refugee and asylum service in Lambeth and Southwark. The client group have complex health and social care issues, so you must have an understanding of their health needs and be confident when managing risk and safeguarding. You will be working alongside an experienced inclusion health team which includes GPs, AHP's and nurse practitioners. This is an opportunity to consolidate your assessment skills and develop your understanding and knowledge of this client group.

The Health Inclusion Team works across Lambeth, Southwark, and Lewisham to provide a high quality comprehensive specialist nurse service for clients who have difficulty accessing primary care services e.g. refugees and asylum seekers, homeless clients, and clients with addictions and high users of A&E. The Health Inclusion Team aims to assist these clients to access mainstream health services.
Key services provided are:
- Assistance with GP registration,
- Health screening (i.e. TB, blood borne virus screening, sexual health screening, baseline routine blood tests)
- Comprehensive health assessment
- Minor illness / injury management
- Chronic disease management
- Harm reduction work
- Wound care
- Case management of complex clients
- Referral to other services as appropriate

Detailed job description and main responsibilities
The post holder will run health screening clinics independently and jointly with other team members. All nurses within the team will at times work across all the areas of homelessness, refugees, asylum seekers and addiction clients.
The post holder will be covering other types of accommodation for asylum seekers such as hotels for asylum seekers, day centres and specialist GP services.
