An exciting opportunity has arisen within the Division of Intermediate Tier Services, rated OUTSTANDING by the CQC, we are seeking an advanced clinical Practitioners to join the Extensive Care service, predominately to work within our Acute frailty service this is a blend of Community and Acute hospital work, within our new acute frailty unit.
Extensive Care is an Umbrella term as the service has a number of sub services within it.
These are, an intermediate bed base within the Stamford Unit, this is a 96 bedded Unit over 3 floors. This is a blended approach to care with Therapy GP and an ACP workforce. We are also very proud to have are new Urgent Crisis response team that work across our Neighbourhoods and support our Award-winning Digital Health Service, Virtual wards, along with having very strong ties to our intermediate urgent care team, this is a truly integrated team consisting of Nursing, Therapy and Adult social care. The Extensivist service is well established over 5 years, and sees patient in the community who have multiple hospital attendances, along with long term conditions and complex health needs. We are really excited to be developing our acute frailty service and Community frailty service

The Advanced Clinical Practitioner (ACP) will work in partnership with the Extensive Care Service, Crisis Response Team, and Frailty Same Day Emergency Care (FSDEC), across Tameside and Glossop locality. In addition to maintaining a professional portfolio, the post holder will demonstrate
the four pillars of advanced practice, enhanced levels of judgement, discretion and autonomous decision-making whilst working alongside the multi-professional framework for advanced practice in England. The post holder will offer clinical expertise, guidance, and management plans to complex frail patients in the community and secondary care setting. The post holder will supervise and drive improvements in treatment quality, motivating colleagues through change process and clinical audits,
evidence-based practice, mentorship, development of protocols and pathways and professional clinical management and leadership.
The post holder will work within a Multi-disciplinary Team (MDT) of Advanced/Enhanced Clinical Practitioners and support from specialist consultants and General Practitioners to ensure consistency in care plans, delivery of care and management to complex patients, diagnostic and treatment interventions within evidence-based practice. This role will advocate collaborative working with both primary and secondary care providers.

Detailed job description and main responsibilities
SECTION 1- MAIN RESPONSIBILITIES:

The Post Holder will:
- Work as an Advanced Practitioner making independent clinical judgements/decisions based on patients clinical need
- Act as lead for the development of advanced and specialist practice
- Undertake independent comprehensive assessment and management of patients to incorporate physiological and pharmacological considerations, making multifaceted complex clinical decisions regarding patient management and clinical outcomes
- Demonstrate a high level of competence in clinical, diagnostic and examination skills
- Management of patients / and carers within the community with complex care needs.
- Provide counselling and advocacy for patients with acute and chronic conditions Undertake research and service development.
- Establish close liaison and communication between all stakeholders across health and social care, community, primary care and third sector disciplines involved in the care of patients and their families and work in partnership with other practitioners.
- Monitor and evaluate the role of the Advanced Practitioner in liaison with appropriate managers and general practitioners, and demonstrate and respond to changing needs
- Develop and sustain close working relationships, partnerships and networks and draw upon multi agency/inter-professional resources where appropriate in order to improve health
PROFESSIONAL RESPONSIBILITIES:
- Keep up to date with developments in the role of the Advanced Practitioner through personal professional development, in service training and education, professional conferences, courses and seminars as appropriate
- Keep accurate records and compile reports as necessary
- Maintain high professional standards of care and expertise
- Be involved in the production of written protocols, clinical guidelines, standards and documentation systems for own use and use by other
- Maintain and extend professional knowledge and skills to keep up to date with current trends and practices
- Be proactive in the accessing of inter-professional clinical supervision
MAIN RESPONSIBILITIES:
THE POST HOLDER WILL BE EXPECTED TO:
• Demonstrate the four pillars of advanced clinical practice.
• Proactively support the development of the ACP role primarily focusing on older people living with frailty in the community, positively impacting on length of stay and the quality of patient care.
• Actively participate in professional leadership and education across the nursing/allied health professionals, medical and therapy teams.
• Act as a role model of standards of behaviour and professionalism, through commitment to the integration of policies and procedures within role and workplace.
• Engage in collaborative efforts with other essential professionals to support local and regional clinical networks and specialty-specific groups.
• Analyse relevant local, regional, and national policy, within area of expertise, advise on impact for service, leading the implementation of changes and evaluation of processes in partnership with the MDT.
• Maintain professional registration and practice through CPD.
• Maintain professional standards of conduct and behaviour consistent with Trust and associated Regulatory Body guidelines.
• Allocate, coordinate, monitor and assess own workload and that of individuals or the service, ensuring resources are deployed appropriately.
• Take a proactive role in shaping the development of current and future service pathways, whilst engaging with appropriate internal and external stakeholders.
• Showcase financial literacy by making sure resources are acquired and used appropriately to satisfy service/service user demands within a given budget.
• Participate in the implementation and delivery of the standards set within the Trust ‘s Quality Strategy.
• Promote, champion, and actively participate in key Trust initiatives.
