We have an exciting opportunity for an enthusiastic and dynamic individual to join our Integrated Discharge Team as a Band 4 Senior Discharge Co-ordinator working collaboratively with internal and external partners, actively monitoring and case managing the discharge plans for patients, with complex care and support needs.
The ability to work autonomously, but also as part of a team, alongside a good working knowledge of hospital discharge (D2A), is essential to the role, alongside excellent communication skills, and the ability to remain calm under pressure.
If successful you will work as a Discharge Coordinator within an established team, based onsite at the NNUH, as part of the wider Integrated Discharge Team – please note we are unable to support hybrid working, you will be ward based, working in a patient environment.
You will need the ability to work effectively and confidently as part of both the ward team, and IDT, excellent organisational skills are essential and the ability to prioritise a conflicting workload.
Discharging our patients to the right destination in a timely manner is key to maintaining flow throughout the Norfolk and Norwich University Hospital (NNUH), we advocate a “home first” ethos, initiating discharge planning from the point of admission, supporting early conversations, and early referrals to ensure patients are discharge ready, at the point of no longer having criteria to reside.

- To have an up to date knowledge of the multidisciplinary management of the patients and discharge plan of all patients referred on D2A P1-3 , alongside complex case support . Expectation is to have ability to manage more complex discharge cases with support of Complex Discharge Nurses.
- To have a good understanding of Discharge to assess, Home first and Discharge Pathways 1-3, as identified in the hospital discharge and community support guidance.
- To have an understanding of clinical conditions and terminology
- To independently gather and collate information from the medical notes, patients and multi-disciplinary team colleagues to enable a clear plan for discharge to be followed from point of admission, gathering evidence to support a safe and timely discharge.
- To provide updates, co-ordinate discharges and provide administrative support to the multi-disciplinary team within the hospital and system partners to improve joint working practices leading to more effective patient care and timely discharges.
- To ensure ward view, OPTICA and handover remain up to date with any discharge progress.
- To assist in ensuring that all patients have an accurate EDD identifying if they meet Criteria to Reside and reason for delayed discharge reason in line with National Discharge policy.
- Using the medical notes and discharge plan and the expected date of discharge, consider how the process of care will be integrated for each individual patient and how a reduction in length of stay can be achieved.

Travelling to work
Staff parking is available onsite; however, this is limited during core working hours. We have more capacity in the evening and at weekends and can usually provide spaces during those hours.
We look to allocate these spaces fairly. Our Travel Office have a set of criteria that staff must meet in order to qualify. If you qualify for a space, you may be placed on a waiting list if we are at full capacity.
The NNUH is served by a number of public buses, and we offer generously discounted monthly passes with First Bus & Konect.
The Costessey Park and Ride runs regularly each day from 06:30 (from P&R) to 20:45 (from NNUH) and this service is free for NHS staff. There is a regular Thickthorn P&R service as well which is free. Applications to the Travel Office are required for this service. We also have a discounted service from the airport P&R if you purchase a bundle. Please see Konect website for further details.
The NNUH host multiple bicycle racks and a salary sacrifice scheme. We also benefit from a Beryl station onsite.
