Come and join our dynamic Adult & Older Adult Community Mental Health Team! We are looking for dedicated individuals who are committed to transforming community mental health services for our aging population. You will be working alongside a well established MDT including, Social workers, Mental Health nurses, psychologists, pharmacists and many other AHP’s who will support you in delivering excellent holistic care.
Crewe is well known for its extensive transportation links and thriving live music scene as well as a range of amenities that make it an up-and-coming town to work in. Crewe is undergoing extensive redevelopment and investment which goes further to improve the facilities available to the public.
The post is also aligned to a thriving network of Care Community groups that support each other to tackle health inequalities, pool collective knowledge and services whilst also streamlining access to our respective areas
The successful candidate will be provided with extensive professional and clinical supervision frameworks and multiple opportunities to develop their specialist clinical Skills and professional skills through an extensive and well resourced CPD structure.

Undertake holistic assessments and coordination of care of service users. Ensuring that communication with the service users / families / and GP's is of the highest standard. To develop and maintain positive working relationships with service users and their carers’, General practitioners, third sector partners, other significant professionals and members of the multi-disciplinary team. Ensuring that clear channels of communication are established and maintained. To promote and identify areas requiring development within the Community Mental Health Team and participate in the development of services in conjunction with the Team Manager, head of clinical services and other professionals.
