What is a Foundation Trust?
South Warwickshire University NHS Foundation Trust was formed on 1 March 2010. A foundation trust status gives us greater freedom to manage our own affairs, and makes us more directly accountable to local people through our Members, who are drawn from the local population and staff.

Probationary Period
All new staff to the Trust will be subject to a standard 6 month probationary period, the details of which will duly follow in your contract of employment.

We are seeking a dynamic and dedicated pharmacist to join our medicines optimisation team. This is an exciting opportunity to contribute to the delivery of medicines value, formulary management and homecare services within our trust.
The successful candidate will have the opportunity to work with the homecare delivery service model, supporting the growth and management widening their experience with different service models and delivery. The post holder will have the opportunity to partake in projects relating to delivery of Cost improvement Programmes. The post holder will be encouraged to work towards the independent prescribing qualification where not already obtained, to support cost effective prescribing and switching.
In order to keep their clinical skills current, the post holder will be expected to spend some of their time providing clinical pharmacy work cover. This will be allocated as part of their job plan and may vary according to clinical service requirement.
This is an excellent and unique opportunity to develop leadership skills and work with multi-disciplinary teams to deliver high quality care to patient and participate in high-value cost efficiency projects.

To support the pharmacy management of medicines value and homecare on a day-to-day basis together with the Lead Pharmacist – Medicines Optimisation at South Warwickshire NHS Foundation Trust (SWFT).
Promote the cost effective prescribing of medicines in line with Trust formulary and medicine value schemes supporting the biosimilar and generic implementation at SWFT to maximise Cost Improvement Programme delivery.
Support the delivery of the homecare medicines service model at SWFT to ensure seamless and smooth delivery whilst working closely with the Lead Technician and Pharmacist for Medicines Optimisation.
To contribute to the strategic development and business plan of pharmacy services for the medicines optimisation, medicines value and homecare.
To contribute to the provision of routine clinical Pharmacy services to other areas of the Trust, and to undertake routine department based duties when rostered including the Dispensary.
To assist with education and training for medical, nursing and pharmacy staff, both undergraduate and postgraduate, including supervising designated junior pharmacists, foundation trainee pharmacists, technicians and (senior) assistants.
To provide a high level of clinical pharmacy care to patients on allocated wards, to support the medical, nursing and other healthcare staff providing clinical care in the specialty areas, and to assist in the clinical and cost-effective use of drugs.

Role Specific Duties
Support the Lead Pharmacist - Medicines Optimisation for the delivery of
medicines value, formulary management and Homecare.
Apply the Medicines Optimisation Principles of the Carter Report
Work proactively with the Lead Pharmacist - Medicines Optimisation and
clinical teams to support development, monitoring and implementation of
Cost Improvement Programmes
Support the delivery of a medicines value schemes including high cost drug
and low cost high volume drugs
Be a liaison and facilitator for wider drug related Cost Improvement
Programmes in other specialist areas working with the respective lead
pharmacists
Act as a Pharmacist Independent Prescriber in a relevant specialty, or
across specialities for a specific group of medicines such as biosimilar
Acts as a role model for prescribing choice change
Support the growth and development of the homecare service in
conjunction with the lead pharmacist
Work to ensure that homecare services meet the recommendation so the
Hackett report, the RPS Professional Standards for Homecare Services,
and the recommendations of the National Homecare Medicines Group.
Partake in the homecare clinical screening rota to timely process homecare
prescriptions
Work in collaboration with clinical teams to resolve queries or issues raised
regarding homecare prescriptions or the service provision
Work with procurement teams to facilitate national generic contract
changes and liaise with lead pharmacists where further specialist advice
required
Ensure compliance to Blueteq Hi-Cost approval for high cost drugs and
work with finance and clinical teams to reduce any losses of income related
to lack of Blueteq approval forms
Work with clinical teams and the Lead Pharmacist – Medicines
Optimisation to ensure high-cost drugs are introduced following
commissioning and trust governance
Participate in the development and update of high cost drug biologic
pathways within agreed specialist areas
Work with partners including dispensary, clinical teams and pharmacists to
communicate decisions related to medicines optimisation and homecare
Provide specialist advice to other pharmacists and clinical teams regarding
homecare and high-cost medicines
Support the principal and lead pharmacist to adhere with the trust formulary and maintenance of the trust formulary in accordance with local policy and agreed decisions
Deputise for the Lead Pharmacist - Medicines Optimisation where
necessary in internal and external meetings
General Pharmacist Duties
To provide a high level, responsive, and developing clinical pharmacy
service to individual patients on allocated wards, and assist junior
pharmacists in such roles, and ward-based Medicines Management
Technicians in their duties.
To be actively involved in efficient discharge planning on the allocated
wards by reviewing and transcribing patient’s discharge treatment (as
appropriate) facilitating timely dispensing of discharge medication and,
where necessary, liaising and communicating with relevant healthcare
practitioners in intermediate or primary care to ensure safe transfer and
continuity of care.
To lead the efficient organisation and smooth running of a dispensary,
including assuring that the environment is ready and safe for the day’s work
To monitor service delivery
To provide support to the Outpatients dispensary operated by SWFT
Clinical Services Ltd. on demand from Main Dispensary as appropriate.
To train and support staff to maintain formulary control, and adherence to
prescribing and dispensing policies, and to collect prescription charges
To assist the Lead and Senior Technician in policies, procedures and the
framework for dispensing, and the planning and development of the service
including automation and service control
To liaise with patients and answer their questions and concerns about their
therapy, and to provide tailored counselling around complex regimes
To advise and support ward and clinic staff in all aspects of medication
usage including clinical aspects, safety & governance and technical
aspects such as controlled stationery, service hours, late prescribing, e-prescribing, and use of the tracker to monitor prescription progress
To undertake clinical checks of the treatment regimens in care pathways,
and raise and resolve anomalies with prescribers,
To contribute to dispensing
To ensure that staff log errors and near misses and that the team and the
individual learn from these
To pick up issues arising during weekend service hours and address these
on the next full working day
To assist and supervise as necessary, ward–based Medicines
Management technicians in the clinical aspects of their work,
To provide clinical and final checks of bought-in specials,
To maintain an awareness of current developments in pharmacy practice
pertinent to the specialist pharmacy area.
When required, to advise on potential improvements in the clinical and cost effective use of drugs in the specialised pharmacy area.
To contribute to and/ or undertake audit projects in the specialty areas
particularly those concerned with the clinical and cost-effective use of
drugs, compliance with safer prescribing and drug administration policies,
procedures and guidelines, and implementation of pharmacy service developments. To provide reports and present the audit findings within the
Trust.
To provide clinical pharmacy training for undergraduate pharmacy
students, vacation students, foundation trainee pharmacists (hospital and
community-based) and hospital clinical pharmacists.
To contribute to the late night, weekend, Bank Holiday and Statutory Day
working rota.
If requested, to be an authorised signatory for drug purchase orders and
for drug invoice payments.
To contribute on rota to the Emergency Duty Commitment of pharmacists
providing an out-of-hours emergency Pharmacy service.
To demonstrate and maintain a commitment to continuing professional
development and encourage other members of the team to do likewise.
To supervise and mentor more junior staff in the provision of services to
these areas
To develop or contribute to a training programme for clinical pharmacists
allocated to the specialty areas and to administer any competency
assessment process that is in place to ensure the appropriate standards
are achieved.
To contribute as required to the induction and in-service training of nursing, midwifery and medical staff as appropriate
To undertake duties as a clinical pharmacist to other wards in the Trust
when necessary, including occasional work at other pharmacies from which
a pharmacy service is provided.
Other Duties
Any other duties considered appropriate to the post and grade by the line
manager.

Special Requirements of the Postholder
The post holder will have Warwick Hospital as their main base but will be
required to work at and/or visit other hospitals in the Trusts we serve.
