NHS Frimley Integrated Care Board was formed on 1 July 2022 under the Health and Care Act 2022, replacing NHS Frimley Clinical Commissioning Group.

The ICB is the new statutory NHS organisation responsible for planning and delivering health and care services. It will work collaboratively with partner organisations including the voluntary, community and social enterprise sector, people and communities across the Frimley Health and Care Integrated Care System (ICS).

Detailed job description and main responsibilities

The postholder will have responsibility for:

Leading and delivering a medicines optimisation service to care homes and social care settings.

Working with local clinicians and other staff to identify, develop and implement initiatives to improve services for the local population in care homes and social care.

Improving the quality and safety of medicines processes in care homes and social care settings.

Designing and delivering specialist education events on medicines optimisation and prescribing issues for care home and social care staff.

Implementing the clinical governance agenda in relation to medicines optimisation to improve or maintain the quality and cost effectiveness of prescribing.

Recommending, facilitating and making sustained changes in prescribing practice to ensure high quality, safe, evidence-based and cost-effective medicines use within care homes.

Providing an expert clinical service to care homes and social care settings including complex structured medication reviews.

Supporting and developing PCN and GP practice pharmacy staff to provide structured medication reviews.

Working as part of a multi-disciplinary and multi-organisation team to deliver medicines related improvements in care.

Work collaboratively across the healthcare system, with care home, social care, local authority and other multidisciplinary teams to support and optimise medicines use.

Interdisciplinary working with other clinicians across the healthcare system to ensure evidenced based clinical guidelines are implemented.

Identifying and reporting safeguarding concerns to the appropriate teams.

Contributing to the development and delivery of both Medicines Optimisation initiatives and Chief Medical Officer directorate initiatives.

Acting as a role model by demonstrating inclusive behaviours, valuing diversity, and highlighting inequalities.

· Deliver on a wide range of activities and projects, as identified by the post-holder’s line manager.

You must have appropriate UK professional registration.

Qualifications

Essential criteria

Registered with the GPhC as a pharmacist
Active engagement with relevant continuing professional development
Educated to Post-graduate Diploma or Master’s level; or demonstrable equivalent level of experience working at a senior level
Post-graduate qualification in clinical or advanced pharmacy practice or equivalent; or demonstrable experience of working in a clinical role
Qualified as an Independent Prescriber or planning to undertake the qualification in the near future
Line management training or experience in line management
Aware of competencies set out in Advanced Pharmacy Framework and working towards them

Knowledge and experience

Essential criteria

Knowledge of NHS policy both locally and nationally Knowledge of and care for the communities of Frimley ICS.
Recent clinical or education experience in the NHS.
Knowledge of the NHS, its structures and processes.
Broad clinical and pharmaceutical knowledge.
Experience of supporting education and training in clinical settings.
Track record of influencing and motivating others.
Experience of working with care homes or other social care teams.
Knowledge of current medicines optimisation policies in the NHS.
Experience of designing and delivering education or training sessions to health care professionals and care staff.
Experience of successfully working as part of a multi-disciplinary or multi-organisation team.
Experience and understanding of developing and implementing change.
