The Norfolk and Norwich University Hospital (NNUH) is a 1,200-bedded teaching hospital with state-of-the-art facilities for modern patient care: 1100 beds in the PFI and 100 in Cromer Community Hospital.
The NNUH is part of Norfolk and Waveney University Hospitals Group which includes the James Paget Hospital, in Great Yarmouth and the Queen Elizabeth Hospital in King Lynn.
We carry out nearly one million outpatient appointments, day case procedures and inpatient admissions annually. The population we serve is predominantly the people of Norfolk and north Suffolk, although some patients are referred from further afield especially to access specialist services available at this Trust.
Our staff of c10,000 care for and support patients who are referred to us by around 100 local GP practices and from other acute hospitals. We run more than 60 specialist services as a tertiary centre, including more than a dozen specialist cancer and radiotherapy treatments, and we are one of the biggest cancer centres in the country.
We provide complex spinal surgery services, specialist Neonatal Intensive Care, neurosciences services and specialist cardiac services. Our Trust runs specialist dermatology, rheumatology, endocrinology, adult congenital heart disease services, treatment of complex liver and pancreatic diseases in adults, cystic fibrosis services and intestinal failure services. We are enhancing our urgent and emergency care specialist services for patients with significant injuries as a result of trauma and have an ambition to become a Major Trauma Centre. NNUH is currently the busiest Trauma Unit in the East of England caring for almost 850 trauma patients annually many with complex care needs.

Job overview
The Clinical Nurse Specialist (CNS) in End-of-Life Care provides expert guidance and support to patients, families, and healthcare staff in managing the complex needs of patients nearing the end of life. Assessing patients’ and family’s needs holistically and providing support at end of life focussing on ‘what matters most’ to them & their support network. This role focuses on delivering high-quality, compassionate care that respects the dignity & wishes of the patient & their family. The CNS acts as a leader in end-of-life care, offering education, developing care plans & ensuring the integration of best practices in end-of-life care across the healthcare continuum.
The End-of-Life care team is part of the Palliative Care Department. The teams work closely with their medical colleagues whilst working autonomously managing their own clinical caseloads, participating in the delivery of education, and undertaking audits & service improvement projects. The clinical caseloads are varied which reflects the diversity of the patient population of the Trust and enables close working with the many specialties that make up our unique Trust. The End-of-Life Care team works in close partnership with several specialised teams, including the Butterfly Volunteers, the Bereavement Team, Mortuary Services, the Spiritual Healthcare Team & the Fast Track Team. Together they ensure that patients and their families receive holistic, compassionate & seamless care during the final stages of life.

Main responsibilities of the role include:
- Clinical Expertise: Provide advanced clinical care and consultation in managing symptoms, pain, and emotional distress for patients nearing the end of life.
- Patient and Family Support: Offer support and guidance to patients and families, helping them navigate the challenges of end-of-life care, including decision-making and advance care planning.
- Education and Training: Develop and deliver educational programs for nursing staff and other healthcare professionals on palliative and end-of-life care practices on the wards and in a classroom setting when required.
- Care Coordination: Collaborate with interdisciplinary teams and outside charities and support services to create and implement individualised care plans that align with the patient's wishes and best practices in end-of-life care.
- Advocacy: Advocate for patient rights and ensure that care delivered respects the cultural, spiritual, and personal values of the patient and their family.
- Quality Improvement: Lead initiatives aimed at improving the quality of end-of-life care services, including the development and implementation of new protocols and care models
- To deliver education based on the 6 Ambitions for palliative and end of life care and the 5 Priorities of care

Detailed job description and main responsibilities
- To manage a specified case load of patients, providing information and support, to enable patients to make informed choices about their care and understand the rational of investigations and treatments.
- To work closely with the Consultants, medical and nursing staff, multi-disciplinary team and Heads of Department within the Directorate.
- To ensure that patients are seen within recommended time scales and targets where these are in place.
- To visit inpatients as appropriate to ensure continuity of care and to give advice/ support.
- To accept referrals from medical/nursing staff both within and outside the Trust, of newly diagnosed and existing patients
- To periodically review caseload and to discharge patients appropriately within divisional guidelines
- To respond to telephone enquiries and co-ordinate admission or changes in appointment dates as warranted by the patient’s clinical condition, including operation of a telephone helpline for the relevant speciality if required.
- To undertake a comprehensive assessment of the patient’s symptoms and in liaison with the Medical team organise investigations as required.
- To supply and/or administer medication according to Patient Group Directions or in accordance with the Trust’s Prescribing Policy for registered nurse prescribers.
- To communicate sensitive condition related information effectively with patients and carers in a timely and understandable manner, using different communication methods to clarify understanding.
- To liaise with other Specialists Nurses, Consultants, GPs, Dieticians, other members of the MDT, voluntary agencies, National charities, other agencies within and outside the Trust.
- To work as an autonomous practitioner responsible for managing and prioritising own workload against the needs of patients and service requirements
- To assist the Line Manager in the provision of clinical and managerial leadership and support of all nursing and clerical staff

Travelling to work
Staff parking is available onsite; however, this is limited during core working hours. We have more capacity in the evening and at weekends and can usually provide spaces during those hours.
We look to allocate these spaces fairly. Our Travel Office have a set of criteria that staff must meet in order to qualify. If you qualify for a space, you may be placed on a waiting list if we are at full capacity.
The NNUH is served by a number of public buses, and we offer generously discounted monthly passes with First Bus & Konect.
The Costessey Park and Ride runs regularly each day from 06:30 (from P&R) to 20:45 (from NNUH) and this service is free for NHS staff. Applications to the Travel Office are required for this service.
