Job overview

The post holder will ensure that the team provides therapeutic care within a holistic philosophy for those experiencing mental health problems. They will hold responsibility for the service users under designated consultants. They will provide; coordination of home treatment, holding knowledge of the relevant service users, a point of contact to CMHT/inpatient ward, planning service user’s pathway including facilitating early discharge from wards, ensuring that safety/follow up plans are in place before discharge from service. Nurturing ward and CMHT relationships will be key in this post, including greater utilization of 3rd sector to aid longer term recovery.
The post holder is responsible for care delivery and maintaining standards of care and clinical practice in liaison with the wider multi-disciplinary team. This will include reviewing documentation (particularly care plans/risk assessments) and collating feedback from service users and carers on discharge, sharing learning from this feedback and implementing improvements based on it.

The post holder will be able to step in and support with daily running tasks of the band 6 in the CRHTT.
Work with the team band 7 to collaboratively audit team paperwork to drive improvements across the service.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Detailed job description and main responsibilities
Clinical:
· Ensuring that all work duties and tasks are performed in keeping with he trust values (please refer to page one)
· Works to the remit of professional role and responsibilities within a multidisciplinary team
· In partnership with other professionals provide excellent and specialist service user/client care, this should include assessment, planning, implementation and evaluation of care in accordance with UHB policies and co-production principles
· Assess service users and carers’ needs for appropriate community or hospital-based care, promote maximum independence through preventing social isolation and coordinating services to support individuals to remain at home where possible
· Undertake clinical risk assessment and formulate and implement risk reduction strategies, promote safety planning and coproduction risk management/safety plans
· Implement home treatment care for an assigned group of service user/clients based upon agreed assessment/care planned
· Provide clinical advice as required to all staff groups, carers and junior colleagues
· Ensure that nursing documentation is maintained to NMC and UHB standards (including electronic records)
· Hold case management responsibility for service users who meet the criteria for the Crisis Resolution and Home Treatment Team, be responsible for assessing individual needs, planning care, implementing care consistently, and evaluating the outcomes of care. Support team in managing higher risk or complex individuals, undertake joint assessments/visits if appropriate to develop others confidence and skills
· Establish and maintain therapeutic relationships with service users in keeping with the policy and ethos of the Service. Provide any prescribed treatments at home or in other crisis service setting (crisis house/crisis recovery unit), including medication and psycho-social interventions, promoting health education and recovery at every opportunity
· Assist the service user and carer by supporting and educating, enable carers to provide continued support and care. Identify support needs of carers and facilitate assessments/agencies that can assist them. Involve carers in all aspects of care planning and assessment, valuing their input and role
· Participate in Locality ward rounds/bed flow meetings, promote early discharge options and facilitate earlier use of home treatment in order to support discharge from hospital where appropriate
· Make referrals to Locality teams, 3rd sector and/or other relevant agencies as part of effective holistic care planning and goal setting
· Administer medications in accordance with NMC standards and local protocols, monitor, record and report therapeutic/adverse medication effects relaying this information to responsible clinician.
· The post holder will be able to step in and support with daily running tasks of the band 6 in the CRHTT.
Communication and Liaison:
· Work closely with crisis assessment ward, designated locality ward and locality CMHT’s to deliver efficient and effective crisis services across locality
· Ensure open channels of communication with both internal and external agencies
· Possess excellent communication skills and be able to form a positive therapeutic relationship with service users/clients in the context of coproduction, partnership and trust
· Be aware of how verbal and non-verbal communication can be interpreted by others and ensure a polite approach is maintained at all times
· Help promote Team cohesiveness, multidisciplinary working, and at all times work collaboratively with colleagues in all parts of the UHB to achieve healthy and effective communication
· Work cohesively with all members of the team in ensuring that the very best services to service users/clients/carers are provided at all times.
· Ensure that all views of service user/clients are effectively sought channelled and acted upon, this may include dealing with concerns in accordance with the UHB policy
· Ensure that information of a confidential nature is not divulged to any unauthorised person and is stored in line with the provisions of the Data Protection Act
· Ensure that all documentation is legible and written in accordance with professional guidelines and best practice
· Demonstrates familiarity with IT skills in order to produce reports and update databases
· Actively participate in the communication of sensitive/difficult information and subsequent support of service users/clients, relatives, carers and colleagues as appropriate
· Undertake discharge reviews gathering feedback on the service user/carers experience, demonstrate learning from this and ensure it is shared with the team
· Maintain key relationships within the Locality including; primary care, CMHT and inpatient services to facilitate communication lines that improves service user pathways/experience
· Participate in Locality business meetings and relevant CMHT forums, support earlier crisis intervention/home treatment to prevent hospital admissions
Management/Leadership
· Assist and support the Team Leader in the establishment of an efficient working environment making the most effective and efficient use of staff, supplies and services and ensuring standards are met
· Work within agreed levels of resources both human and financial
· Actively engage with self-rostering and adopt a flexible approach to personal rostering to ensure the needs of the service are met, the postholder is expected to work largely during working week/day shifts to establish consistency and links with CMHT though a degree of flexibility is expected
· To provide a safe and attractive working environment for team members within available resources
· Delegate tasks and activities to a range of team members in relation to service user/client’s care for designated locality
· Record and report all incidents/concerns involving staff, service users/clients and visitors in accordance with UHB policies and assist in any investigations as required
· Participate in conducting nursing documentation audits to achieve the agreed standards
· Exercise sound management skills based on effective organisation and good working relationships, being proactive in the provision of an effective and efficient service, assisting with the provision of care or management responsibilities in the absence of colleagues
· Be aware of all aspects of health and safety and the health and safety of the team members in terms of undertaking clinical risk assessments, implementing safe environments and reporting hazards
· Support and implement the Quality & Safety agenda for the Directorate, including monitoring and maintaining professional standards, participating in Clinical Audit and ensuring the reporting of complaints and highlighting concerns over patient care
· Contribute to specific local Service Developments, ensure all team members have a real voice in the development of the services and opportunities to be actively involved
· Implements procedures related to stock control, taking responsibility for checking adequate equipment available for effective functioning of service
· Actively manage own and others annual leave in line with UHB and local Policy and Procedure
Personal and Professional development
· Promote an up to date and appropriate service which provides a positive image and good clinical practice through personal development and development of team
· Participate in personal development opportunities, maintain and develop knowledge and skills through the appraisal process, and keep professionally updated at all times, undertaking duties in accordance with best practice and NMC requirements. Maintain a practice portfolio evidencing this
· To access clinical supervision at regular intervals and to take responsibility for identifying his/her own support needs and liaise with manager
· Participate in staff education and development, including orientation and supervision and act as a mentor, preceptor and a resource for learners and the nursing team.
· The post-holder is responsible for ensuring all relevant Trust Policies and Procedures are followed to the identified standard for clinical audit and that their professional conduct and clinical practice is in line with NMC professional requirements
· Promote a positive image of the UHB at all times; acting as an ambassador/role model for the organisation and the nursing profession
· There is potential of staff Rotations with equivalent grades within the Crisis service and Locality to aid personal development
