Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. We provide a full range of hospital and community services for people in south London and as well as specialist care for patients from further afield including cancer, renal, orthopaedic, respiratory and cardiovascular services.
Guy’s is home to the largest dental school in Europe and a £160 million Cancer Centre opened in 2016. As part of our commitment to provide care closer to home, in 2017 we also opened a cancer centre and a kidney treatment centre at Queen Mary’s Hospital in Sidcup. St Thomas’ has one of the largest critical care units in the UK and one of the busiest emergency departments in London. It is also home to Evelina London Children’s Hospital.
Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.
In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. Our merger provides a once in a generation opportunity to build a lasting, world-renowned heart and lung centre, providing the highest quality care for patients and conducting world-leading research.

An exciting opportunity has arisen for a motivated, dynamic and proactive nurse to join our ACHD team at Guy's and St Thomas' Hospital.
We are looking for a nurse with a strong background in caring for adult cardiac patients, ideally with experience in the care of adults with
congenital heart disease (CHD ) and an interest in transition and CHD surgery/intervention.
The successful candidate will provide expert clinical skills, knowledge, support and advice to patients with CHD and their relatives as they move through outpatient and inpatient services and across the network.
The role will include taking responsibilities for the ongoing clinical management of patients in nurse-led and multidisciplinary clinics, during in-patient episodes and in the community through liaison with local health providers. You will also provide specialist support and advice to healthcare professionals within the Trust.
The successful candidate will be expected to work both autonomously and as part of a specialist multidisciplinary team. Excellent communication and organisational skills are essential for this role. The role will involve off site travelling to support outreach clinics.
There will be ongoing support provided by the ACHD consultants and nurse specialist team.
The candidate will need to provide evidence of post-registration study.

The post holder will develop, implement and evaluate a seamless specialist service, ensuring that patients receive the highest standard of clinical care. They act as a lead specialist and will demonstrate a high level of expertise within the specialist service, providing specialist advice, education and support to health care professionals and patients. The post holder will actively undertake clinical audits and research within the specialist area.
The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues.
The post holder will also ensure the safety and well being of services users, and always working in a manner that promotes dignity and human rights through the adoption of person-centered care principles.

of 16 years. The service is closely aligned to the paediatric cardiology service which is based in the Evelina London Children's Hospital (ELCH),
particularly in ensuring the streamlined transition of patient care from children's to adult services.

As an adult congenital heart disease nurse specialist the role will include:
- Providing clinical support and advice lines to adult congenital heart disease patients/careers (Telephone/ Email advice lines).
- Supporting the review and management of in-patients, contributing to the care plan and communicating and providing support to the patients and their relatives.
- Acting as resource for healthcare professional within the Trust and across the network.
- Running face-to-face and virtual clinics for specific patient groups (e.g. transition nurse led clinics).
- Pre-assessing and providing education to patients undergoing intervention and surgery.
- Collaborating with anaesthetics in regards to patient safety and planning of care for complex cases requiring cardiac and non-cardiac surgery.
- Attending and contributing in MDT meetings.
- Providing specialist education and training to nurses including students nurses, midwives and allied health professionals to assist them to care for patients with congenital heart disease.
