We are looking to appoint a female support worker due to the needs of the service

This is a excellent opportunity to be part of delivering care to new and existing patients that require Physical Health Monitoring within the South Dales Treatment and intervention team at the Goodall Centre, Bishop Auckland. You will not only be part of a Physical Health Team but part of the wider community teams based in the Goodall Centre.
The main role of this post is to completed generally physical observations, BP, Pulse etc, also ECG's and venepuncture. Experience and skills in this aspect are essential to the role but training can be provided for the right candidate. The physical Health clinic and functioning is supported by the leadership team.
As part of the Physical Health Team you will attend the daily Huddles and MDT, liaise with prescribers and the colleagues in the team to ensure safe practice and care.
Other aspects of the role include working with patients to provide Psychological wellbeing interventions and with the support and supervision of qualified staff.

The weekly tasks will involve the organisation and running of the Physical Health Clinic along with your colleagues, you will be required to book, and follow up appointments. Maintain accurate patient records and spreadsheets to ensure robust monitoring of patient medication.
Where home visits are required you will organise these by liaising with the patients, carers and others.
The Physical Health Team take responsibility for the clozapine clinic which runs every Tuesday, you will be provided with specialist training to be able to operate and maintain the Pochi Machine. The Clinic works closely with the pharmacy staff to ensure that the patients on clozapine received safe and appropriate care.
Candidates must have the following qualifications to apply for the post
Diploma in Health and Social Care level 2 or equivalent
Care Certificate
Key skills in Literacy and Numeracy level 2
Successful completion of Trust’s full HCA Framework
within 6-month probationary period

Trust Welcome Day
Here at Tees, Esk and Wear Valley Foundation NHS Trust we want you to have a positive experience when you start your new role with us. To ensure you have great welcome, for all new starters joining the Trust, we offer two start dates per month on the 1st and 3rd Mondays of that Month.
On your start date, you will be invited to our in person Trust Welcome Day at our Flatts Lane Centre, Middlesborough. Your first day will be engaging and informative, it will allow you to meet and network with a range of Trust staff, learn more about us and what we offer. For certain clinical roles, your Welcome Day could then be followed by up to 9 days face to face training. If this is applicable to you, further information will be provided ahead of your Welcome Day.
