Are you looking for an exciting Challenge ,then if so then the borough of Lewisham is for you,we are  currently looking for a Learning Disability CHC Nurse Assessor to work in our exciting team,you who will be responsible for those patients who meet the eligibility criteria for NHS Continuing Healthcare.
This role is based in Catford in  Lewisham  but the individuals will be expected to travel to out of borough placements. The post holders will work as part of the continuing care team to develop the role and to contribute to the development and implementation of a robust NHS continuing healthcare process and assessment for NHS funded care.
The individuals will support effective multidisciplinary team working and the development of positive multi agency working.

As strong advocates you will ensure that the care of NHS continuing healthcare patients reflects best clinical standards and patients are treated with dignity and respect.

The individual will have the knowledge and experience to challenge traditional clinical practices to ensure that evidence based best practice is identified and implemented. This may include supporting the implementation of new roles and extended clinical practice to modernise the service to benefit the care of patients and their families.

In line with most NHS organisations in London, we do not pay travelling expenses for interviewees. In exceptional cases we would consider reimbursing expenses but interviewees must get agreement from the recruiting manager before attending the interview.
