Do you enjoy supporting people with Personality Disorder? Do you have experience of facilitating groups?
The Bucks Primary Care Mental Health Hub (PCMHH) is a new and innovative service, which has been set up as part of delivering the Community Mental Health Framework (CMHF) objectives. The team brings together a range of health care professionals and support staff, to provide care and treatment to patients in their own communities.
We aim to deliver more proactive and responsive care to reduce
unnecessary hospital admissions, secondary mental health service referrals, or repeat GP appointments wherever possible. PCMHH Teams aim to offer a collaborative approach to deliver care for people with multiple long-term conditions by providing better, local and faster access to services for patients at an earlier point in the care journey.
PCMHH Teams offer careful, holistic assessments based on a biopsychosocial care model. SUN (Service User Network) is a community support group for people with a personality disorder or complex emotional needs. SUN provides regular community-based peer support groups offering an opportunity for people to share experiences, resources and support one another in a non-judgemental and understanding environment. SUN sessions are facilitated by a Senior clinician and a
specialist Peer Support worker who has lived experience of Personality Disorder.

The SUN service model draws upon therapeutic community principles and works closely with the wider specialist personality disorder services in Buckinghamshire.
The SUN is a very supportive and experienced clinical team based within the Bucks PCMHH. SUN has proved a popular resource for many people in Bucks and as such, we are recruiting to this post to support our plans to expand and further develop the service.
You will work closely with the SUN Lead Facilitator and experienced Peer Support Workers to deliver the SUN weekly programme of facilitated peer support groups across the county of Buckinghamshire. These groups are a mixture of community[1]based face to face groups and an online group.
Additional duties include clinical screening of referrals, co-ordinating member involvement meetings, promoting SUN across services and providing line management for peer support workers in the team.
