North London Hospice is a registered charity, that has been caring for local people since 1984.  The Community teams cover three London boroughs Barnet, Enfield, and Haringey 

We are now looking for a Clinical Nurse Specialist (CNS) to join our community team as a one year secondment. The CNS contributes to the provision of specialist and end of life care in the community for patients diagnosed with life limiting illnesses and their informal carers. Working with the MDT, accepting patients to agreed criteria and managing a shared caseload .  They will provide advice, education and support to patients, their carers and other health and social care professionals.  They will contribute to the development and smooth running of the service. The post holder will work to agreed professional, department and Hospice boundaries as required to provide patient centered seamless care and take part in the 7 day / week service to community patients.
