Job overview
A post has arisen for a Band 7 Diabetes Specialist Nurse to work within the Integrated Cornwall Diabetes Specialist Nursing Team, in the North/East of the County. The team has close liaison with the community, secondary care providers & primary care with the aim of providing a seamless journey for the diabetes patients. We are looking for a Diabetes Specialist Nurse who is enthusiastic about the field of diabetes, wanting to improve & make a difference of patients in the community. Previous diabetes expertise is essential with proven evidence of working within a Diabetes Team.
The successful candidate will be expected to
- To work as an autonomous advanced practitioner and effectively deliver an expert integrated service within the locality area, supporting community services, hospitals, primary care and secondary care
- To manage a caseload of patients/clients via nurse led Intermediate clinics at Community hospitals and GP cluster locations. Provide Community hospital in-patient visits
- To ensure that patients and their families are fully informed and supported by providing clinical and social support
- To lead in the development of the team and evidence based practice in line with current research and guidelines
- To establish clear lines of communication within the specialty team to ensure cohesive multidisciplinary management of patients
- Perform the role of educator to student dieticians, medics, nursing staff, dietetic assistant and other AHP’s as require
Advert
To provide a specialist nursing service and education to people with Diabetes, their carers, and healthcare teams across community, primary and secondary care settings within North / East Cornwall. This will include managing a patient caseload, acting as an educational resource for service users / providers & promoting effective communication between all health professionals and the voluntary sector.To assist with professional leadership and management of the Cornwall Adult Diabetes Nursing Service in conjunction with the Lead Clinical Nurse Specialist to provide the highest possible standards of care to the patients and their families.
Nurse led Intermediate clinics are run by the post holder at Community hospitals and GP cluster locations. The post holder also provides Community hospital in-patient visits where required, and support to Cornwall Foundation Trust HealthCare Professionals

Eduction/Qualification and Relevant Experience
Essential criteria
- Professional qualification – RN level 1
- Diabetes specialist course (ENB 928 or Diabetes degree module)
- Relevant first degree or evidence of working towards this.
- Substantial experience at a senior level within an adult care acute / community setting caring for people with Diabetes
Desirable criteria
- Independent Non- Medical Prescribing Course, or willingness to undertake
